Análise do potencial angiogênico da terapia fotodinâmica antimicrobiana em ratas tratadas com dose oncológica de Zoledronato

Resumo: Bisphosphonates (BPs) are potent drugs, with antiresorptive properties, used for treatment of some bone diseases or orthotropic malignancies. A serious adverse effect caused by the use of BPs is bisphosphonate-related osteonecrosis of the jaw (BRONJ). Vasculature involvement is claimed as one of the possible pathogenetic factors of BRONJ, however, existing studies are inconclusive, inconsistent and contradictory. As the etiology of this pathological condition is poorly understood, there is great difficulty in preventing and successfully treating of this disease. One of the few preventive therapeutic approaches is antibiotic therapy, which has proven to be unsuccessful. Antimicrobial photodynamic therapy (aPDT) has concurrently shown antimicrobial capacity and bioestimulation abilities, for the vasculature as well, which places it as a potential preventive therapeutic strategy for BRONJ. The objectives of this study were as follows: to analyze vasculature involvement on the pathogenesis of BRONJ in rats that showed the main risk factors for developing the disease and evaluate the effect of aPDT on the vasculature when applied to the tooth extraction site. Thirty senile rats (20 months old) were divided into four experimental groups: SAL, ZOL, SAL/aPDT and ZOL/aPDT. Drug treatment plan lasted seven weeks. The rats received intraperitoneal injections of 0.45 ml of 0.9% sodium chloride solution (Groups SAL and SAL/aPDT) or 0.45 ml of the same solution plus 100 ug/kg zoledronate (Groups ZOL and ZOL/aPDT) with an interval of two days between applications. After three weeks of treatment all rats were subjected to dental extraction of the first lower left molar. Um grave efeito adverso ocasionado pelo uso dos BPs é a osteonecrose dos maxilares associada à terapia medicamentosa (ONM-M). O envolvimento da vasculatura é apontado como um dos supostos fatores etiopatogênicos da ONM-M, todavia os estudos existentes são inconclusivos, incongruentes e contraditórios. Como a etiopatogenia desta condição patológica é pobremente compreendida há grande dificuldade na prevenção e no tratamento da doença. Dentre as poucas propostas terapêuticas preventivas está a antibioticoterapia, a qual se mostra falha. A terapia fotodinâmica antimicrobiana (aPDT) apresenta concomitantemente capacidade antimicrobiana e ação bioestimulatória, inclusive sobre a vasculatura, o que a coloca como uma potencial estratégia terapêutica preventiva para a ONM-M. Os objetivos do presente estudo foram: analisar o envolvimento da vasculatura na etiopatogenia da ONM-M em ratas que apresentavam os principais fatores de risco para o desenvolvimento da doença e avaliar a ação da aPDT sobre a vasculatura quando aplicada no sítio de extração dental neste modelo experimental. Trinta ratas senis (20 meses de idade) foram distribuídas em quatro grupos experimentais: SAL, ZOL, SAL/aPDT e ZOL/aPDT. O plano de tratamento medicamentoso teve duração de 7 semanas.
