Osteonecrose relacionada aos bifosfonatos

Resumo: | Bisphosphonates are the drugs used in osteoporosis, bone metastases, multiple myeloma, hypercalcemia and Paget's disease of bone. However, with the growing use of this medication were reports of necrosis of the face's bones related to their use. This study analyzes the cases of two patients seen at the Oral Oncology Center of Univ Estadual Paulista - UNESP, Campus Araçatuba, diagnosed with bone necrosis. In the first clinical case the patient had bone metastases arising from prostate adenocarcinoma. He used bisphosphonates for a year associated with chemotherapeutic agents and corticosteroids. The patient was submitted hyperbaric oxygen therapy and requested transfer to another department where he underwent surgical resection with reconstruction with titaniun plate. In the second case the patient with multiple myeloma took bisphosphonate associated with Thalidomide and Venkad, was referred for evaluation of fistula in lower alveolar bone. On physical examination was noted bone exposure in the region of incisive papilla. The patient was monitored and submitted to a biopsy of the lesion in the mandibular body. Both patients died. Currently there is no consensus about the treatments recommended for bone necrosis associated with bisphosphonates because of this occurrence being observed with greater frequency recently. Therefore, further studies directed towards the development of bone necrosis should be performed in order to give the best care to patients who use bisphosphonates for the treatment of other diseases |

Entretanto, com o emprego crescente dessa medicação houve o relato de necrose de ossos da face relacionado ao seu uso. Este trabalho analisa dois casos clínicos de pacientes atendidos no Centro de Oncologia Bucal da Faculdade de Odontologia do Campus de Araçatuba da UNESP com diagnóstico de necrose óssea. No primeiro caso clínico o paciente era portador de metástases ósseas decorrente de adenocarcinoma de próstata. Fez uso de bifosfonato por um ano associado a agentes quimioterápicos e corticosteróides. O paciente foi submetido à oxigenioterapia hiperbárica e solicitou encaminhamento para outro serviço onde foi submetido à ressecção cirúrgica com reconstrução por placa. No segundo caso o paciente portador de mieloma múltiplo, submetido à bifosfonato associado à Talidomida e Venkad, foi encaminhado para avaliação de fístula em rebordo alveolar inferior. Ao exame físico notava-se exposição óssea em região de papila incisiva. O paciente foi acompanhado e submetido à biópsia da lesão em corpo de mandíbula. Ambos os pacientes foram a óbito. Atualmente não há consenso sobre os tratamentos indicados para necrose óssea associada ao bifosfonato, devido a esta ocorrência ser observada com maior freqüência recentemente.
