Os efeitos da hemodiálise a longo prazo na mobilidade funcional e no equilíbrio postural em pé

Resumo: The prevalence of patients with advanced chronic kidney disease (CKD) needing maintenance hemodialysis therapy is increasing rapidly. Patients with CKD may have changes in the locomotor system. Postural control represents a complex interaction between sensory and motor systems, involving the perception of environmental stimuli, responding to changes in body orientation within the environment to maintain the center of gravity within the support base, therefore, any change in these systems results in loss of postural control. Therefore, the objective of the study was to evaluate the long-term changes in static and dynamic balance in patients undergoing hemodialysis. For that, patients with a clinical diagnosis of renal failure undergoing hemodialysis were selected. The dynamic postural balance was evaluated by the tests: Timed up and Go (TUG), Short Physical Performance Battery (SPPB) and Average walking speed. The static postural balance was evaluated by a force platform. The evaluations were made at two different times: first evalution (T0) and the second evaluation after six months (T1). The comparisons between the values were made using the paired t test or the Wilcoxon test, with a value of p≤0.05 being accepted as significant. There was no significant difference between the variables evaluated for both static and dynamic balance. Thus, we can conclude that hemodialysis did not alter functional mobility and standing postural balance in the long-term basis.

Pacientes com DRC podem apresentar alterações no sistema locomotor. O controle postural representa uma complexa interação entre os sistemas sensorial e motor, envolvendo a percepção a estímulos ambientais, respondendo a alterações na orientação corporal dentro do ambiente para manter o centro de gravidade dentro da base de apoio, sendo assim, qualquer alteração nesses sistemas implica no prejuízo do controle postural. Diante disso, o objetivo do trabalho foi avaliar as alterações a longo prazo do equilíbrio estático e dinâmico em pacientes submetidos a hemodiálise. Para isso foram selecionados pacientes com diagnóstico clínico de insuficiência renal realizando hemodiálise. O equilíbrio postural dinâmico foi avaliado pelos testes: Timed up and Go (TUG), Short Physical Performance Battery (SPPB) e Velocidade média de marcha (VM). O equilíbrio postural estático foi avaliado por uma plataforma de força. As avaliações foram feitas em dois momentos distintos: primeira avaliação (T0) e, após seis meses, a segunda avaliação (T1). As comparações entre os valores foram feitas usando-se o teste t pareado ou o teste de Wilcoxon, com um valor de p≤0,05 sendo aceito como significante. Não houve diferença significativa entre as variáveis avaliadas tanto para o equilíbrio estático, quanto para o dinâmico.
