Análise de dois períodos preemptivos da dexametasona na remoção dos terceiros molares: um estudo clínico randomizado e duplo-cego

Resumo: Surgery for removal of third molars or other included teeth associated with preoperative administration of dexamethasone is very helpful, since it may decrease postoperative patient discomfort. This study aims to compare edema, pain and trismus in the extraction of third molars using two preemptive periods of dexamethasone. Ten patients aged between 18 and 30 years were enrolled in this study. The patients were divided into groups, which underwent extraction of the upper and lower third molars on medication administered in two distinct periods, one of them in a single dose of 8 mg dexamethasone one hour before surgery, and another in a fractionated dose with 4 mg eight hours prior and another 4 mg dexamethasone one hour prior to the procedure. The patients participated in both groups because it was a study with the Split-mouth model. The patients were evaluated at two and seven postoperative days for edema, by means of facial measurements: vertical linear measurement A (outer corner of the eye to the angle of the mandible), horizontal linear measure B (corner of the mouth to the lobe of the ear ); pain through the Visual Analogue Scale (VAS), and trismus by inter-incisal distances. The data were submitted to the 2-factor ANOVA test (pre-emptive groups versus analysis period), and for the relevant interactions the Turkey post-test (p <0.05) was applied. The results showed that measure A was similar between groups and periods (p> 0.05), while measure B was similar between groups, but there was statistical difference between periods (p <0.05), with greater measures on the second day compared to the seventh postoperative day, with the largest measure being 115 mm, the lowest being 90 mm and the mean being 105.3 mm on the second day in group 1. Esse estudo tem como finalidade comparar o edema, dor e trismo na extração de terceiros molares utilizando-se dois períodos preemptivos da dexametasona. Dez pacientes com idade entre 18 e 30 anos fizeram parte deste estudo. Os pacientes foram divididos em grupos, os quais foram submetidos a extração dos terceiros molares superiores e inferiores sob medicação administrada em dois períodos distintos, sendo uma delas em dose única de 8 mg de dexametasona uma hora antes da cirurgia, e outra em dose fracionada com 4 mg oito horas antes e mais 4 mg de dexametasona uma hora antes do procedimento. Os pacientes participaram de ambos os grupos por se tratar de um estudo com o modelo boca dividida. Os pacientes foram avaliados aos dois e sete dias de pós-operatório quanto ao edema, através de mensurações faciais: medida linear vertical A (canto externo do olho ao ângulo da mandíbula), medida linear horizontal B (canto da boca ao lóbulo da orelha ipsilateral); a dor por meio da Escala Visual Analógica (EVA), e o trismo pelas distâncias inter-incisais. Os dados obtidos foram submetidos ao teste ANOVA de 2 fatores (grupos preemptivos versus período de análise), e para as interações relevantes foi aplicado o pós-teste Turkey (p<0,05).
