This cross-sectional observational research study aims at assessing the midpalatal suture (MPS) ossification level in children and adults, evaluating jaw CT scans of patients at various ages. The sample comprised 315 Cone-Bean Computed Tomographies (CBCT), randomly divided into 3 groups: 6 - 12 year-old subjects (103 CT scans), 13 - 18 year-old subjects (105 CT scans) and 19 - 27 year-old subjects (107 CT scans), belonging to the Dental School Radiology Service records at the National University of San Marcos, in Lima, Peru. CT scans were obtained in multifile DICOM file format and, then, imported into PointNix Real Scan 2.0 software, positioning the head of subjects in three planes of space. The axial reconstruction image was set on the Microsoft PowePoint software on a black background for a better observation of the midpalatal suture. At the level of half of the palate, the moments in which the MPS ossified were established and defined in five stages, being represented by score A and B (absence of MPS interdigitation), stage C (beginning of the MSP ossification), stage D (MPS fusion in the palatal bone) and stage E (absence of the MPS in the palatal bone). Both the Kappa Coefficient (0.911; IC 95%: 0.843 - 0.979) and the Weighted Kappa Coefficient (0.933; IC 95%: 0.876 - 0.991) were higher than 0.90. The Chi-Square test results lead to the conclusion that the stage depends on the patient age (p < 0.001). Therefore, considering the total sample, it was observed that 3 subjects (1.0%) were in stage A, 47 subjects (14.9%) were in stage B, 114 subjects (36.2%) were in stage C, 66 subjects (21.0%) were in stage D and 85 subjects (27. 0%) were in stage E. Multiple comparison tests support the conclusion that subjects belonging to the 6 – 12 year-old age group (Median = 3.00; Mean = 2.89; SD = 0.93) presents lower values, both when compared to the 13 – 18 year-old age group and the 19 - 27 year-old age group (p < 0.001). On the other hand, there are no statistically significant differences between the two older age groups (p = 0.840). Thus, it can be concluded that the beginning of the midpalatal suture ossification occurs, significantly, in subjects under 12 years of age. There is no difference in the midpalatal suture fusion in subjects between 13 - 27 years of age. MPS opening shows predictable success until the age of 12 years
