Avaliação microscópica de implante de polietileno poroso de alta densidade utilizado em processo alveolar

Resumo: | The lost of one or more teeth leads to damage of alveolar process due to the varying degrees of bone resorption, depending on the causes or the affected region. Maxilla is especially susceptible to resorption, mainly in the anterior region for its bone characteristics. For local rehabilitation, autogenous bone grafts are usually employed, favoring dental implants insertion, connective tissue grafts for aesthetics purposes, or even prosthetic adaptation. For aesthetic purposes, other materials are being tried, as the high density porous polyethylene (HDPP), especially when filling and contouring of some regions such as chin and zygoma are desired. The present study suggested the use of HDPP in alveolar process as an alternative to autogenous grafts and prosthetic devices. For this, New Zealand rabbits underwent surgical procedure to receive HDPP onlay implants in buccal face of mandibular alveolar process, fixed with titanium screws. After 7, 15, 45, and 90 days, the animals were killed and the specimens retrieved for histological and imunohistochemical analysis. No implant loss or infection was detected at the retrieval of the specimens. The microscopical analysis presented sactisfatory integration of the material to the bone surface, with new bone formation in the receptor bed and inside the pores of the material, observed from the 45th day. Imunohistochemistry showed low expression of Cbfa-1 and moderate of VEGF at all observed periods. These results encourage the indication of use of HDPP in alveolar ridge when aesthetic recovery is needed. |

A maxila é especialmente suscetível à reabsorção, principalmente em sua parte anterior pelas características ósseas que a constituem. Para a reabilitação local, comumente faz-se uso dos enxertos ósseos autógenos favorecendo a inserção de implantes osseointegráveis, de tecido conjuntivo, quando se busca recuperação estética, ou mesmo compensação protética. Para fins estéticos, outros materiais vêm sendo experimentados, como o polietileno poroso de alta densidade (High Density Porous Poliethilene HDPP), principalmente onde se deseja preenchimento de determinadas regiões, como o mento e o malar. O presente estudo propôs a utilização do HDPP em regiões de processo alveolar, como alternativa aos enxertos autógenos e artifícios protéticos. Para tanto, foram utilizados coelhos Nova Zelândia, que receberam implantes de HDPP tipo onlay na face vestibular do processo alveolar mandibular fixados com parafusos de titânio. Após os períodos de 7, 15, 45 e 90 dias, os animais foram sacrificados e os espécimes removidos e submetidos à análise morfológica microscópica e imunoistoquímica. Nenhuma perda ou processo infeccioso foram observados na coleta dos materiais. A partir da análise microscópica, observou-se boa integração do material à superfície óssea, com neoformação óssea no leito receptor e no interior dos poros do material, observado a partir do 45.º dia. A análise imunoistoquímica mostrou baixa expressividade do Cbfa-1 e moderada de VEGF em todos os períodos analisados. Esses resultados encorajam a indicação do HDPP em regiões de rebordo alveolar quando da necessidade de recuperação estética da área.Made available in DSpace on 2015-09-14T10:13:34Z (GMT).
