The use of arm-circumference measurement in the determination of the nutritional status of preschool children

Summary: Use and limitation of a classification of the nutritional status of preschool children based exclusively on arm circumference were studied. Gomez' classification was used as the basis for the determination of nutritional status. Trigger levels of arm circumference were determined for three age groups (2, 3 to 5 and 6 years of age) regardless of sex. Specificity and sensibility of the classification ranged from 75% to 80%. The method could be simplified by using tapes or cords previously marked with the trigger level appropriated for each age group. Potential applications of the arm circumference were discussed and it was concluded that possibly the best use of the arm circumference would be in the screening of the nutritional status of whole population of children.

Em população de pré-escolares de baixa renda, no interior do Estado de São Paulo, Brasil, foram avaliadas as possibilidades e as limitações de uma classificação do estado nutricional baseada exclusivamente na tomada do perímetro braquial. Utilizando a classificação de Gomez como referência para o diagnóstico da desnutrição, foram determinados níveis críticos para a medida do perímetro braquial que identificariam o estado nutricional de pré-escolares com 75-80% de sensibilidade e especificidade. São propostos, como simplificação na utilização do perímetro braquial, o agrupamento dos pré-escolares em apenas três grupos de idades (2, 3 a 5 e 6 anos completos) e o emprego de três fitas ou cordões, onde previamente fossem demarcados os níveis críticos correspondentes a cada um dos grupos. Discutem-se as aplicações possíveis da classificação baseada no perímetro braquial de onde se conclui que o seu uso mais promissor estaria na averiguação do perfil nutricional de conjuntos de pré-escolares.
