Resumo: Introduction: Bipolar affective disorder (BAD) is a complex psychiatric condition characterized by extreme mood swings, including episodes of mania and depression. The relationship between alcohol and drug use and the risk of suicide in patients with BAD has been the subject of study due to its clinical significance and impact on mental health. Methods: A review was conducted using the PubMed, PsycINFO, Scopus and Scielo, databases, focusing on relevant scientific articles, systematic reviews, and meta-analyses. Investigations regarding the association between psychoactive substance use, bipolar disorder, and suicidal behavior were analyzed. Results: Evidence suggests that the use of alcohol and drugs is significantly associated with an increased risk of suicidal behavior in patients with BAD. Studies indicate that substance abuse can worsen the severity of BAD symptoms, decrease treatment adherence, and complicate clinical management. Furthermore, the presence of psychiatric comorbidities, such as substance use-related disorders, further complicates the picture. The results underscore the importance of comprehensive therapeutic approaches that consider both the clinical aspects of BAD and the potential comorbidity with alcohol and drug use. Early identification and appropriate treatment of substance abuse are essential for reducing the risk of suicidal behavior in patients with BAD. Conclusion: The correlation between alcohol and drug use and the risk of suicide in individuals with BAD is a relevant topic in mental health. Integrative approaches that encompass psychotherapeutic and pharmacological treatment, along with interventions for substance dependence, are crucial for improving the quality of life and safety of these patients.
