Uso preventivo de antifibrinolíticos em cães

Resumo: | The perioperative blood loss is a common and expected consequence during the surgical act. However, depending on the size of the surgery and the patient's comorbidities, the volume lost may exceed what is expected and bring with it several hemodynamic complications, as well as increase surgical morbidity and mortality and the requirement of transfusion therapy. In this scenario, dogs tend to benefit from prophylactic antifibrinolytic therapy, since, besides the complications of excessive bleeding, tissue trauma can promote the occurrence of coagulopathies, being hyperfibrinolysis, although little diagnosed, the most reported in the canine species. Antifibrinolytics include a plasma protease inhibitor called Aprotinin, and lysine analogs represented by Tranexamic Acid (TA) and Epsilon-Aminocarproic Acid (EACA). However, currently therapy with antifibrinolytics still depends on a correct diagnosis of fibrinolytic activity, since problems in other stages of clot formation will not be solved by these drugs. Even with the creation of increasingly sophisticated tests, as is observed in thromboelastography (TEG) and thromboelastometry (ROTEM®), access to this technology is still limited in the routine of veterinary medicine, while there is a need to obtain equipment for a more immediate analysis. Therefore, the administration of antifibrinolytics in these patients is a promising therapy, but it is still largely done empirically. |

Entretanto, a depender do porte da cirurgia e das comorbidades do paciente, o volume perdido pode exceder o esperado e trazer consigo várias complicações hemodinâmicas, bem como aumentar a morbimortalidade cirúrgica e o requerimento de uma terapia transfusional. Neste cenário, cães tendem a se beneficiar de uma terapia antifibrinolítica profilática, visto que, além das complicações de uma hemorragia excessiva, o trauma tecidual pode promover a ocorrência de coagulopatias, sendo a hiperfibrinólise, embora pouco diagnosticada, a mais relatada na espécie canina. Entre os antifibrinolíticos, estão inclusos um inibidor de proteases plasmáticas chamado Aprotinina, e os análogos da lisina representados pelo Ácido Tranexâmico (TA) e o ácido Épsilon-Aminocarpróico (EACA). Contudo, nos dias atuais a terapia com antifibrinolíticos ainda depende de um correto diagnóstico da atividade fibrinolítica, visto que problemas em outras etapas formação do coágulo não serão solucionados por estes fármacos. Mesmo com a criação de testes cada vez mais sofisticados, como é observado na tromboelastografia (TEG) e tromboelastometria (ROTEM®), o acesso a esta tecnologia ainda é limitado na rotina da Medicina Veterinária, e existe a necessidade da obtenção do equipamento para uma análise mais imediatista.
