SPOROTRICHOSIS: EXUBERANT PRESENTATION IN AN ENDEMIC CONTEXT

Resumo: | Introduction: Sporotrichosis is an endemic disease in the state of Rio de Janeiro caused by a Sporothrix spp. fungus, whose most common species is the S. brasiliensis. The illness can manifest in cutaneous or systemic form, more frequently as the cutaneous affection with lesions restricted to the skin, subcutaneous and adjacent lymphatic vessels. Objective: Describing a case of cutaneous lymphatic sporotrichosis, emphasizing the importance of a correct diagnosis, highlighting the epidemiological context, in addition to an appropriate notification. Case report: Male, 47 years old, presented a lesion in the right hemithorax with extension to the dorsal zone, ulcerated, with purulent material outlet, erythema around and palpable, and painful lymph node enlargement. Fungal culture revealed the growth of Sporothrix spp., and the histopathological exam showed a chronic inflammatory process in progress, suggesting infectious dermatitis. Treatment with Itraconazole 200mg / day was initiated, resulting in significant clinical improvement in the first month of treatment. Discussion: Despite the fact that sporotrichosis is known to spread through contaminated soil, a strong relationship between human infection and disease in cats has been observed in recent years. We must highlight the relevance of observing the clinical signs of the lesion to aid diagnosis, especially when the epidemiological history of contact with the feline is negative. Conclusion: The adoption of control and management measures for affected patients and animals is essential in order to minimize the number of cases in the region. |

A doença pode se manifestar sob a forma cutânea ou sistêmica, sendo a cutânea mais comum, com lesões restritas a pele, subcutâneo e vasos linfáticos adjacentes. Objetivo: Descrever um caso de esporotricose cutâneo-linfática, enfatizando a importância de um diagnóstico correto, levando em consideração contextos epidemiológicos, além de uma notificação apropriada. Relato do caso: Paciente masculino, 47 anos, apresentou lesão no hemitórax direito com extensão para região dorsal, ulcerada, com saída de conteúdo purulento, eritema ao redor e linfonodomegalia palpável e dolorosa. Realizada cultura fúngica que revelou crescimento de Sporothrix spp. e o exame histopatológico demonstrou processo inflamatório crônico em atividade, sugestivo de dermatite infecciosa. Foi iniciado tratamento com Itraconazol 200mg/dia obtendo melhora clínica importante no primeiro mês de tratamento. Discussão: Apesar da esporotricose ser conhecida por seu contágio através da terra contaminada, tem-se observado forte relação da infecção humana com a doença em gatos nos últimos anos. Nota-se a relevância da observação dos sinais clínicos da lesão para auxílio do diagnóstico principalmente quando a história epidemiológica de contato com o felino é negativa.
