Localized sporotrichosis in a young feline: case report

Resumo: It is reported a case of sporotrichosis in a mixed breed male feline of four months old and who had no partner or access to the street, in which there was an apparent human  involvement. Edematous, ulcerative lesion with necrotic spots and serosanguineous exudate in the digit of the right thoracic limb was verified. The cytopathological examination revealed the presence of yeast-like cells compatible with Sporothrix sp. The radiographic examination showed osteolysis in the digit mentioned. The isolated administration of itraconazole proved to be effective in the treatment. Complete remission of the lesions occurred within 12 weeks. The tutor was referred for medical care and was duly treated. Sporotrichosis requires a prolonged time of treatment and clarification from owners about the care in handling the animal. The tutors showed resistance to attending clinical follow-up appointments, what probably was aggravated by the positive diagnosis of SARS-CoV-2 in one of them.

El paciente presentaba una lesión ulcerosa edematosa con manchas necróticas y exudado serosanguíneo en el dedo de la extremidad torácica derecha. La citopatología de la lesión reveló la presencia de estructuras levaduriformes compatibles con Sporothrix sp. La radiografía mostró osteólisis en el dedo de la extremidad torácica derecha. El uso aislado de itraconazol demostró ser eficaz en el tratamiento. La remisión completa de las lesiones ocurrió dentro de las 12 semanas. El tutor fue remitido para atención médica y fue debidamente atendido. La esporotricosis requiere un tratamiento prolongado y una aclaración por parte de los propietarios sobre el cuidado en el manejo del animal. En el presente caso, los tutores mostraron resistencia a acudir a las citas de seguimiento clínico, condición que se vio agravada por el diagnóstico positivo de SARS-CoV-2 (COVID-19) en uno de los tutores.Relata-se um caso de esporotricose em um felino, macho, sem raça definida, com quatro meses de idade e que não tinha contactante ou acesso à rua, em que houve aparente acometimento humano. Paciente apresentava lesão edematosa, ulcerativa, com pontos necróticos e exsudato sero-sanguinolento no dígito do membro torácico direito. A citopatologia da lesão revelou a presença de estruturas leveduriformes compatíveis com Sporothrix sp. A radiografia evidenciou osteólise no dígito do membro torácico direito. O uso isolado de itraconazol se mostrou eficaz no tratamento. A remissão completa das lesões ocorreu em 12 semanas. O tutor foi encaminhado para atendimento médico e foi, devidamente, tratado. A esporotricose demanda tempo prolongado de tratamento e esclarecimento de proprietários acerca dos cuidados no manejo do animal.
