SYSTEMIC AND LOCAL FACTORS THAT CAUSE FAILURE IN THE OSSEOINTEGRATION OF DENTAL IMPLANTS.

Resumo: Introduction: the osseointegration is a fundamental condition for a successfull dental implant. According to Branemark, we can define it as a connection made directly between implant and bone. As time passes, with dental implants popularization, it was noted that osseointegration reached frequent failure rates for multifactorial reasons. Objective: to ddress the main factors of systemic and local origin that compromise osseointegration and highlight the causes that affect its functionality. Material and methods: bibliographical surveys were carried out in the BVS, PubMed, LILACS e Google scholar databases, without language restrictions, with filter for the last 20 years, giving preference to more recent articles for a more up-to-date approach to the subject. Results: the failure of osseointegration can be caused by some factors such as: diabetes mellitus, periodontal diseases, osteoporosis, tobaccoism, surgical failure and medicinal products ingestion on bisphosphonates. Conclusion:  an investigative anamnesis makes it easier to promote good osseointegration, as the dentist will have the opportunity to evaluate the appropriate treatment options for each case. The patient needs to follow the prescribed guidelines correctly and continuously, both in the postoperative period and in oral hygiene. The subject is controversial, requiring more research over a longer period of time to obtain greater knowledge.

De acordo com Branemark, podemos defini-la como uma ligação feita diretamente entre implante e osso. Ao longo do tempo, com a popularização dos implantes dentários notou-se que a osseointegração alcançou taxas de insucesso frequentes por razões multifatoriais. Objetivo: abordar os principais fatores de origem sistêmica e local que comprometem a osseointegração e evidenciar as causas que afetam sua funcionalidade. Materiais e métodos: foram realizados levantamentos bibliográficos nas bases de dados BVS, PubMed, LILACS e Google Acadêmico, sem restrições de idioma, com filtro nos últimos 20 anos, focando em artigos mais recentes para uma abordagem mais atualizada do assunto. Resultados: o insucesso da osseointegração pode ser causado por alguns fatores como: diabetes mellitus, doenças periodontais, osteoporose, tabagismo, falha cirúrgica e ingestão de medicamentos à base de bisfosfonatos. Conclusão:  uma anamnese investigativa possibilita maior facilidade para promover uma boa osseointegração, pois o cirurgião dentista terá a oportunidade de avaliar as opções de tratamento adequadas para cada caso. O paciente precisa seguir as orientações prescritas de forma correta e contínua, tanto no pós-operatório quanto na higienização bucal.
