Influence of the use of biposphonates on the dental implants osseointegration

Resumo: Tooth loss is a public health problem that affects millions of people. In this perspective, oral rehabilitation through implantology is an alternative, however, for this treatment to be possible, factors such as influences of systemic diseases and medications used by the patient need to be carefully checked. In this context, a group of drugs widely used are bisphosphonates (BFs), such drugs, widely used to inhibit bone resorption and treatment of malignant neoplasms directly influence the bone formation process. Thus, it is necessary to know the dental surgeon about the effects that such a group of drugs can measure the success rate of dental implants. The aim of the present study was to analyze the influence of the use of BFs in the process of osteointegration of dental implants. A literature review was carried out based on articles found on the platforms: Pubmed / Medline, Lilacs, SciELO and capes journals, from which 31 articles were selected due to their relevance to the topic. Therapy using BFs has proven safe in the literature as long as patients are not subjected to high doses of the drug, as well as do not use it in the intravenous form of administration. The success rate of dental implants proved to be equivalent to patients without treatment, however, we must pay attention to factors such as systemic diseases and deleterious habits that make the dental implant have failure rates regardless of the use of the drugs studied here.

En esta perspectiva, la rehabilitación oral a través de la implantología es una alternativa, sin embargo, para que este tratamiento sea posible, se deben controlar cuidadosamente factores como las influencias de enfermedades sistémicas y los medicamentos utilizados por el paciente. En este contexto, un grupo de fármacos muy utilizados son los bisfosfonatos (BF), fármacos de este tipo, muy utilizados para inhibir la resorción ósea y el tratamiento de neoplasias malignas que influyen directamente en el proceso de formación ósea. Por tanto, es necesario conocer al cirujano dental sobre los efectos que tal grupo de fármacos puede medir en la tasa de éxito de los implantes dentales. El objetivo del presente estudio fue analizar la influencia del uso de BF en el proceso de osteointegración de los implantes dentales. Se realizó una revisión de la literatura a partir de los artículos encontrados en las plataformas: Revistas Pubmed / Medline, Lilacs, SciELO y Capes, de las cuales se seleccionaron 31 artículos por su relevancia para el tema. La terapia con BF ha demostrado ser segura en la bibliografía siempre que los pacientes no estén sujetos a dosis elevadas del fármaco y no lo utilicen en la forma de administración intravenosa. La tasa de éxito de los implantes dentales resultó ser equivalente a pacientes sin tratamiento, sin embargo, debemos prestar atención a factores como enfermedades sistémicas y hábitos deletéreos que hacen que el implante dental tenga tasas de fracaso independientemente del uso de los fármacos aquí estudiados.A perda dentária é um problema de saúde pública que afeta milhões de pessoas. Nessa perspectiva, a reabilitação oral por intermédio da implantodontia é uma alternativa, entretanto, para que este tratamento seja possível, fatores como influências de doenças sistêmicas e medicamentos utilizados pelo paciente precisam ser verificados com atenção. Nesse contexto, um grupo de fármacos bastante utilizado são os bifosfonatos (BFs), amplamente utilizados para inibição da reabsorção óssea e tratamento de neoplasias malignas influenciam diretamente no processo de formação óssea. Assim, faz-se necessário o conhecimento do cirurgião dentista dos efeitos que tal grupo de medicamento pode aferir a taxa de sucesso dos implantes dentários. O objetivo do presente estudo foi analisar a influência do uso dos BFs no processo de osteointegração de implantes dentários. Foi realizada uma revisão de literatura tomando como base artigos encontrados nas plataformas: Pubmed/Medline, Lilacs, SciELO e periódicos capes, dos quais foram selecionados 31 artigos devido sua relevância ao tema. A terapia utilizando BFs mostrou-se segura desde que os pacientes não sejam submetidos a altas doses do fármaco, bem como não o utilizem da forma intravenosa de administração.
