Guidelines for fighting the pandemic: use report in a municipality in Bahia

The pandemic scenario caused by the novel coronavirus (SARS-CoV-2) brought the need for reorganizing health care networks in an attempt to implement appropriate and timely actions to provide the population with a comprehensive and resolutive care. Official estimates indicate that 80% of health problems should be solved in primary care (PC). Considering that, the PC Board of the State of Bahia promoted matrix-based/follow-up action of a small municipality through institutional support (IS) teams, aiming to organize the Chronic Care Line using the guide for coping with the pandemic. The guide was implemented using a three-stage pedagogical strategy: methodology agreement with management and municipal coordinators; weekly meetings to use instruments offered by the guide; and implementation of the care line of patients with chronic conditions in four pilot teams of the municipality. The results indicate flow development and implementation, follow-up instruments for patients with chronic conditions, reference and counter-reference sharing charts, and Telemonitoring Guiding Document. Being a collective construction of actions articulated between IS teams, municipal management, and workers, the guide proved to be a powerful tool in the process of reorganizing municipal network and work process of PC teams in face of COVID-19, serving as a model for other municipalities.
