Access and Adherence to Treatment of Women with Breast Cancer Attended at an Oncology Hospital

Resumo: Introduction: The access to oncology treatment consists in expanding the service to women with the disease, while the adhesion suggests a certain reaction to the treatment from the chronic disease patient. Objective: To investigate the determining factors easing or prejudicing the access and adhesion to the treatment by women with breast cancer attended by an oncology hospital in Teresina, Piaui. Method: A quantitative study based on a questionnaire applied to 50 women regarding the Primary Care Assessment Tools and on the Morisky-Green Test, which investigates the aspects of therapeutic admission and adherence. Results: It was found that 36% of users are 39 to 49 years old, 62% live in other cities of Piaui or other states, and 60% earn up to minimum wage. Regarding the time past between the day when the appointment was scheduled and the day it occurred, 72% of women waited up to one week to be attended at the health institution. It was noticed that 42% of women presented nausea and vomits as one of the main collateral effects involved in clinic procedures. Conclusion: The access rate to oncology center facilities was satisfactory, while adhesion rates point out factors that may influence the permanence in the treatment. It is necessary to specify the access in public policies of health concerning oncology and to prioritize therapeutic interventions in breast cancer control, in the context of policies and programs of oncology care.

Objetivo: Investigar los factores determinantes que facilitan o dificultan el acceso y la adherencia al tratamiento de mujeres con cáncer de mama atendidas en un hospital oncológico ubicado en Teresina-Piauí. Método: Estudio cuantitativo realizado en 50 mujeres que implicando la aplicación de un cuestionario, con base en el Primary Care Assessment Tools y en la Prueba de Morisky-Green que investigan aspectos del ingreso y de la adherencia terapéutica. Resultados: Se constató que el 36,0% de las usuarias tienen entre 39 a 49 años; el 62,0% viven en otros municipios de Piauí u otros estados y el 60,0% reciben hasta un salario mínimo. Con relación al tiempo transcurrido entre la fecha de la programación de la cita y el dia de la cita, el 72,0% de las mujeres aguardaron el plazo de hasta una semana para ser atendidas por la institución de salud. Se observó que el 42,0% de las mujeres presentaron náuseas y vómitos como uno de los principales efectos secundarios implicados con la permanencia en procedimientos clínicos. Conclusión: La tasa de acceso a las dependencias del centro oncológico fue satisfactoria, mientras la de adherencia indica factores que pueden influir en la permanencia en el tratamiento. Se hace necesario especificar el acceso a las políticas públicas de salud en oncología e incluir la adherencia a intervenciones terapéuticas como prioridad en el control del cáncer de mama, en el contexto de políticas y programas de asistencia oncológica.Introdução: O acesso ao tratamento oncológico consiste na ampliação do atendimento a mulheres com a doença, enquanto a adesão sugere um comportamento da portadora de doença cronica diante do tratamento. Objetivo: Investigar os fatores determinantes que facilitam ou dificultam o acesso e a adesão ao tratamento de mulheres com câncer de mama, atendidas em um hospital oncológico situado em Teresina-Piauí. Método: Estudo quantitativo realizado com 50 mulheres, envolvendo a aplicação de um questionário, com base no Primary Care Assessment Tools e no Teste de Morisky-Green, que averiguam aspectos do ingresso e da aderência terapêutica. Resultados: Constatou-se que 36,0% das usuárias tem entre 39 a 49 anos; 62,0% residem em outros municipios do Piauí ou outros estados e 60,0% recebem ate um salário mínimo. Em relação ao tempo decorrido entre a data da marcação da consulta e o dia da consulta, 72,0% das mulheres aguardaram o prazo de até uma semana para serem atendidas na instituição de saúde. Percebeu-se que 42,0% das mulheres apresentaram náuseas e vômitos como um dos principais efeitos colaterais envolvidos na permanência em procedimentos clínicos. Conclusão: A taxa de acesso às dependências do centro oncológico foi satisfatória, enquanto de adesão indica fatores que podem influenciar a permanência no tratamento.
