Cancer Cachexia in Oncology Patients Hospitalized in an Interdisciplinary Home Care Program

Resumo: Cancer has been the second largest death cause in Brazil within the last years. Cancer cachexia is a syndrome that contributes to a worse quality of life, as well as higher morbidity and mortality. This study described cachexia occurrence in patients admitted to an Interdisciplinary Oncology Homecare Program from the Hospital Escola/Universidade Federal de Pelotas as for sex, age, body mass index, type of cancer, staging, presence of metastases and disease related symptoms. Cancer cachexia has been identified through the presence of anorexia, weight loss, lean mass and body fat, detected by the Subjective Global Assessment produced by the patient. 108 patients were evaluated within the period between February 2006 and February 2008. Prevalence of cachexia at admission was 46% and the cumulative incidence within this period was 25%. A total of 65 patients had cancer cachexia, corresponding to 60% of the sample, 55% of which were male, whose average age was 59 and mean of body mass index was 21 kg/m². 66% had advanced disease (stage IV) and 85% had metastases. As for the site, 26% had cancer in the digestive system and 25% had lung cancer. In the bivariate analysis, it was observed that the mean scores of symptoms and physical examination as well as the total score of the Subjective Global Assessment Produced by the Patient were directly related with the presence of cachexia. In this group of patients, nutritional and/or drug interventions  in order to alleviate the symptoms can be effective in reducing the occurrence of cachexia.

La caquexia neoplásica es un síndrome que contribuye para una baja calidad de vida, mayor morbilidad y mortalidad. Esta investigación describió la ocurrencia de la caquexia en pacientes internados en el Programa de Internación Domiciliaria Interdisciplinar Oncológico del Hospital Escuela de la Universidad Federal de Pelotas, Brasil, tomando en consideración sexo, edad, índice de masa corporal, tipo de cáncer y su extensión, presencia de metástasis y síntomas relacionados con la enfermedad. La caquexia cancerosa está marcada por la presencia de anorexia, pérdida de peso, masa magra y de adiposidad corporal, detectadas por medio de la evaluación subjetiva global producida por el paciente. 108 pacientes fueron evaluados, desde febrero de 2006 hasta marzo de 2008. La prevalecía de caquexia en la internación fue de un 46% e la incidencia cumulativa en el periodo fue de un 25%. De los 65 pacientes con caquexia, los cuales representaban el 60% de la muestra, un 55% eran del sexo masculino, con promedio de edad de 59 anos e promedio de índice de masa corporal de 21 kg/m2. El sesenta y seis por ciento presentaron estadio IV y un 85% presentaron metástasis. Con respeto a la localización del tumor, un 26% presentaron cáncer en el tracto digestivo y un 25% en el pulmón. En el análisis bivariado se observó que el promedio de la puntuación de los síntomas, del recuento del examen físico y la puntuación total de la evaluación subjetiva global producida por el paciente estuvieron directamente asociados con la presencia de caquexia. En el grupo de pacientes estudiados, intervenciones nutricionales y/o medicamentosas con el objetivo de aliviar los síntomas podrían tener efecto en la reducción de la ocurrencia de la caquexia.O câncer apresenta-se no Brasil como a segunda maior causa de morte nos últimos anos. A caquexia neoplásica é uma síndrome que contribui para uma pior qualidade de vida, maior morbidade e mortalidade. Este trabalho descreveu a ocorrência de caquexia em pacientes internados no Programa de Internação Domiciliar Interdisciplinar Oncológico do Hospital Escola/Universidade Federal de Pelotas em relação a sexo, idade, Índice de Massa Corporal, tipo de câncer, estadiamento, presença de metástases e sintomas relacionados à doença. A caquexia cancerosa foi caracterizada pela presença de anorexia, perda de peso, de massa magra e de gordura corporal, detectadas através da Avaliação Subjetiva Global Produzida pelo Paciente. Foram avaliados 108 pacientes, no período de fevereiro de 2006 a fevereiro de 2008. A prevalência de caquexia na internação foi de 46% e a incidência cumulativa no período foi de 25%. No total dos 65 pacientes caquéticos, que correspondem a 60% da amostra, 55 % eram do sexo masculino, com idade aproximada de 59 anos e média de Índice de Massa Corporal de 21 Kg/m2. Sessenta e seis por cento apresentavam estadiamento IV e 85% eram portadores de metástases. Quanto à localização do tumor, 26% apresentaram câncer no aparelho digestivo e 25% no pulmão. Na análise bivariada, foi observado que a média de pontuação dos sintomas, do escore de exame físico e a pontuação total da Avaliação Subjetiva Global Produzida pelo Paciente estiveram diretamente associadas à presença de caquexia.
