RELATIONSHIP OF DIABETES MELLITUS TYPE II TO DEPRESSION AND TREATMENT WITH ANTIDEPRESSANTS

Resumo: The association between type 2 diabetes mellitus (DM2) and the presence of depression has been presented in the literature by several authors, the apparent symptoms related to hyperglycemia, such as excessive tiredness and weakness may have positive points for the onset of depressive symptoms, as well as responsibility for treatment, dietary restrictions and adverse effects in the use of antidepressant drugs. Among those that stand out as risk factors commonly associated with the development of depressive symptoms in patients with diabetes are: age, obesity, as well as the increase in the amount of drugs in use, such as antidepressants. Thus, this study is a systematic review, and seeks to better understand the relationship between diabetes mellitus and depression and the possible implications of the use of antidepressants on metabolic changes, specifically, glycemic level. Through the analyzed articles, it can be considered that there is a relationship between diabetes mellitus and an increase in clinical depressive symptoms. This relationship covers both metabolic control and adaptive aspects, such as socioeconomic factors, non-acceptance of the disease, low educational level, lifestyle, family history. Antidepressants of the selective serotonin reuptake inhibitor (SSRI) class have been shown to have a positive effect on the treatment of depression in patients with DM2. The most studied antidepressants in this class were Sertraline and Fluoxetine and they were effective in treating depression and improving glycemic control.

The most studied antidepressants in this class were Sertraline and Fluoxetine and they were effective in treating depression and improving glycemic control.A associação entre o diabetes mellitus tipo 2 (DM2) e a presença de depressão tem sido apresentada na literatura por diversos autores, os sintomas aparentes relacionados à hiperglicemia, como o cansaço excessivo e a fraqueza podem ter pontos positivos para o início dos sintomas depressivos, assim como a responsabilidade do tratamento, as restrições alimentares e os efeitos adversos no uso dos medicamentos antidepressivos. Dentre os quais destacam-se como fatores de risco comumente associado ao desenvolvimento do quadro depressivo em pacientes portadores do diabetes estão: idade, obesidade, bem como o aumento da quantidade de medicamentos em uso, como os antidepressivos. Dessa forma, o presente estudo enquadra-se como uma revisão sistemática, e busca compreender melhor sobre a relação da diabetes mellitus com a depressão e as possíveis implicações do uso de antidepressivos sobre as alterações metabólicas, especificamente, nível glicêmico. Através dos artigos analisados, pode-se considerar que existe uma relação entre o diabetes mellitus e aumento dos sintomas depressivos clínicos. Esta relação abrange tanto no controle metabólico quanto em aspectos adaptativos, como, fatores socioeconômicos, a não aceitação da doença, baixa escolaridade, estilo de vida, histórico familiar. Observou-se os antidepressivos da classe dos inibidores seletivos da recaptação da serotonina (ISRS) apresentam um efeito positivo no tratamento da depressão em pacientes portadores do DM2. Os antidepressivos dessa classe mais estudados foram a Sertralina e a Fluoxetina e os mesmos foram eficazes no tratamento da depressão e uma melhora do controle glicêmico.
