Cortisol levels in patients presenting generalized chronic periodontitis and diabetes mellitus

Resumo: Purpose: This study evaluated cortisol levels in healthy subjects, patients with generalized chronic periodontitis (GCP) and diabetes mellitus (DM) type 2, and patients with GCP without DM. Methods: Fifty-nine subjects were selected and categorized into: 1) Control group (n=19, age: 36.4±5.1 years old), 2) Case 1 group (n=11, age: 54.2±19.6 years old) comprising subjects with GCP and DM type 2, and 3) Case 2 group (n=29, age: 40.9±4.4 years old) comprising subjects with GCP. The clinical parameters assessed were: gingival index (GI), probing pocket depth (PPD), probing attachment level (PAL), cortisol levels in plasma, and blood glucose. Results: The clinical parameters PPD and PAL and cortisol levels were statistically different (P=0.01) between the control group and groups Case 1 and Case 2. There was no significant difference between Case 1 and Case 2 groups. Conclusion: Subjects with GCP and DM type 2 showed significant higher cortisol levels than healthy subjects. However, the presence of diabetes did not worsen periodontal disease. Key words: Cortisol; periodontal disease; diabetes mellitus

Key words: Cortisol; periodontal disease; diabetes mellitusObjetivo: Este trabalho avaliou o nível de cortisol sangüíneo em sujeitos saudáveis, sujeitos com periodontite crônica generalizada (PCG) apresentando diabetes mellitus (DM) tipo 2 e sujeitos com PCG e sem DM. Metodologia: Foram selecionados 59 pacientes de ambos os gêneros, sendo um grupo Controle saudável (n=19, idade média de 36,4±5,1 anos), um grupo Caso 1 apresentando PCG e DM tipo 2 (n=11, idade média de 54,2±19,6 anos) e outro grupo Caso 2 apresentando PCG (n=29, idade média de 40,9±4,4 anos). Os parâmetros clínicos observados foram: profundidade de bolsa à sondagem (PBS), nível de inserção clínica (NIC), taxa de cortisol sangüíneo e glicemia. Resultados: Os parâmetros clínicos PBS e NIC e a taxa de cortisol sangüíneo apresentaram diferenças estatísticas significantes (P=0,01) entre o grupo controle e os grupos Caso 1 e Caso 2, mas não houve diferença significante entre os grupos Caso 1 e Caso 2. Conclusão: Pacientes apresentando PCG e com DM tipo 2 não compensada têm aumento significativo no nível de cortisol em comparação a pacientes saudáveis. No entanto, a presença de DM tipo 2 não agravou a doença periodontal.
