Evaluation of the physical examination performed by last year medical students and residents of the Internal Medicine Service of Hospital São Lucas – PUCRS [Abstract in English]

Resumo: Objective: In this study we evaluate medical charts for certain segments of the physical exam as performed by last year medical students and medical residents in patients admitted to the Internal Medicine Service of the Hospital São Lucas, PUCRS. Methods: Following a pre-determined protocol we evaluated retrospectively since january 2000 until march of 2002, 120 randomly chosen charts. Results: Even though certain segments of the exam are registered in over 90% of the cases (cardiovascular, pulmonary and abdominal examinations), the exam was considered incomplete in all the charts. The neurological exam is an example of a particularly serious problem we identified in this evaluation. Even in patients with neurological symptoms, the exam is not performed or registered adequately. Certain segments of the exam such as lymphnodes, thyroid, fundoscopic, otologic, rectal, breast and testicular examination are registered in an absolute minority of the cases. Conclusion: We feel that lack of orientation and role models among the teaching staff is the main reason that explains this situation. Objetivo: Neste estudo avaliamos o registro de determinados segmentos do exame físico realizado por acadêmicos do 6º ano e médicos residentes em pacientes internados na unidade de Medicina Interna no Hospital São Lucas da PUCRS. Métodos: Seguindo um protocolo previamente estabelecido, foram avaliados retrospectivamente 120 prontuários escolhidos de forma aleatória, entre o período de janeiro de 2000 a março de 2002. Resultados: Embora alguns segmentos do exame são registrados em mais de 90% dos casos (exame cardiovascular, pulmonar e abdominal), o exame é incompleto em todos os prontuários. O exame neurológico exemplifica um problema particularmente sério que identificamos nesta avaliação. Mesmo em pacientes que apresentam sintomas ou queixas neurológicas o exame não é realizado ou registrado adequadamente. Alguns segmentos específicos como exame de linfonodos, tireóide, fundoscopia, otoscopia, toque retal, testículos ou mamas são registrados somente numa minoria absoluta dos casos. Conclusão: Entendemos que a falta de modelo e orientação por parte do corpo de professores é o principal motivo que explica esta situação. A necessidade de reverter esta realidade, desenvolvendo estratégias eficazes é imperativa.
