O objetivo deste trabalho foi o de reunir e relacionar a informação mais recente e relevante sobre a fibromialgia. A sua prevalência parece ser de 2 a 8% da população. A sua etiologia é provavelmente multifatorial e a teoria mais bem aceite sobre o seu mecanismo fisiopatológico é a da sensibilização central, onde as áreas matriciais da dor no cérebro são mais fortemente ativadas que em outros indivíduos por diversos estímulos. Ela parece ter também uma forte componente genética. Outros fatores psicológicos e biológicos podem estar na sua origem, como os abusos sofridos na infância e os distúrbios do sono. O seu diagnóstico é sobretudo clínico e a sua sintomatologia inclui não só a dor generalizada mas também uma miríade de outros sintomas, de entre os quais os mais relevantes são a fadiga, os sintomas cognitivos, os distúrbios do sono e a depressão. O seu tratamento deve incluir, para obter eficácia máxima, uma componente farmacológica e uma componente não farmacológica. A terapia cognitivo-comportamental e o exercício aeróbico são as modalidades não farmacológicas com maior evidência. Dentro da terapia farmacológica, os fármacos mais eficientes parecem ser os antidepressivos, particularmente os antidepressivos tricíclicos e os IRSN, e os antiepiléticos, particularmente a pregabalina. Outros fármacos e complementos dietéticos promissores estão em processo de investigação.Fibromyalgia is a chronic disease, of uncertain etiology, uncertain diagnosis, a rather variable phenotype and with an heterogenic treatment. The objective of this work was to compile and relate the most recent and relevant information about fibromyalgia. Its prevalence ranges from 2 to 8% of the general population. The etiology of this disease seems to be multifactorial and the most accepted theory for his physiopatological mechanism is central sensitization, where the matricial pain zones of the brain are strongly activated, in comparison with other individuals, by diverse stimuli. Fibromyalgia seems to have a strong genetic component. Also, other psychological and biological factors may be involved in its origin, like child abuse and sleep disturbances. The diagnosis is most of all clinical and the symptomatology includes not only generalized pain as well as other symptoms, between each, the most relevant are fatigue, cognitive symptoms, sleep disturbances and depression. The treatment should include, for optimized effect, non-pharmacological and pharmacological components. Cognitive behavioral therapy and aerobic exercise are the non-pharmacological therapies with stronger evidence. Within the pharmacological therapy, the most efficient drugs seem to be antidepressants, particularly tricyclic antidepressants and NSRI, and antiepileptics, particularly pregabaline.
