Coffee consumption in hypertensive people: systematic review and meta-analysis

Resumo: Introduction: Hypertension is a major cause of death in the world, it is urgent to intervene in their risk factors for prevention and treatment. The associationbetween coffee intake and hypertension has made health professionals advise against him. Objective: To evaluate the effect of coffee consumption in systolic and diastolic blood pressure in adults and elderly with hypertension. Methods: The systematic review of the literature with meta-analysis following the principles proposed by the Cochrane Handbook was performed. The critical analysis, extraction and synthesis ofdata were carried out by two investigators separately and meta-analyses were performed using RevMan 5.3.5. Results: Three Randomized Clinical Trials (RCT) and twocohort studies covering 264 and 1919 individuals were included respectively. The results of meta-analysis, which included the RCT indicates that the ingestion of coffee with reduced Hydroxyhydroquinone (HHQ) has beneficial effect on systolic blood pressure (MD= -2.60; 95% Cl=-4.81, -0.39; p=0.02) and diastolicblood pressure (MD= -1.30; 95% Cl=-1.67, -0.93; p<0.01). Other studies have shown that the in adult population with hypertension, coffee consumption doesnot interfere with blood pressure. However consumption of more than three cups of coffee a day is associated with the risk of hypertension. In elderlysubjects with hypertension, consumption of more than three cups of coffee increases blood pressure and the possibility of uncontrolled blood pressure. Conclusion: Consumption of more than three cups of coffee a day is not recommended in people with hypertension. Coffee intake with reduced HHQ is advised.

La asociación entre la ingesta de café y la ETS ha llevado a los profesionales de la salud a desaconsejarla. Objetivo: Evaluar el efecto delconsumo de café sobre la presión arterial (PA) sistólica y diastólica en adultos y ancianos con hipertensión. Métodos: Se realizó una revisión bibliográfica sistemática con metaanálisis que siguió los principios propuestos por el Manual Cochrane. El análisis crítico, la extracción y la síntesis de datos fue realizado por dos investigadores por separado, el metanálisis se llevó a cabo utilizando el software RevMan 5.3.5. Resultados: Se incluyeron tres ensayos clínicos aleatorizados (ECA) y dos estudios de cohortes, que cubrieron 264 y 1919 sujetos, respectivamente. Los resultados del metanálisis, que incluyó el ECA, indican que la ingesta de café con hidroxihidroquinona reducida (HHQ) tiene un efecto beneficioso sobre la PA sistólica (DM = -2,60; IC 95% = -4,81, -0,39; p = 0,02) y en PA diastólica (DM = -1,30; Cl 95% = -1,67, -0,93; p <0,01). Los estudios restantes muestran que en la población adulta con HTA, el consumo de café no interfiere con la PA, sin embargo, el consumo de café de más de tres tazas por día se asocia con el riesgo de HTA. En ancianos con HTA, la ingesta de café superior a tres tazas aumenta la PA y la posibilidad de PA descontrolada. Conclusione: En personas con HTA no es recomendable consumir más de tres tazas de café al día. A associação entre a ingestão de café e a HTA tem feito com que os profissionais de saúde o desaconselhem. Objetivo: Avaliar o efeito do consumo do café na Pressão Arterial (PA) sistólica e diastólica em pessoas adultas e idosas com HTA. Métodos: Foi realizada uma revisão sistemática da literatura com metanálise que obedeceu aos princípios propostos pelo Cochrane Handbook. A análise crítica, a extração e a síntese dos dados foi efetuada por dois investigadores isoladamente, a metanálise foi realizada com recurso ao software RevMan 5.3.5. Resultados: Foram incluídos três Ensaios Clínicos Randomizados (RCT) e dois estudos de coorte, abrangendo 264 e 1919 indivíduos, respetivamente. Os resultados da metanálise, que incluiu os RCT, indicam que a ingestão de café com Hidroxihidroquinona (HHQ) reduzida apresenta um efeito benéfico na PAsistólica (MD= -2.60; 95% Cl=-4.81, -0.39; p=0.02) e na PA diastólica (MD= -1.30; 95% Cl=-1.67, -0.93; p<0.01). Os restantes estudos demonstram que na população adulta com HTA o consumo de café não interfere com a PA, contudo o consumo de café superior a três chávenas por dia está associado ao risco de HTA. Nos indivíduos idosos com HTA a ingestão de café superior a três chávenas aumenta a PA e a possibilidade de PA descontrolada. Conclusão: Nos indivíduos com HTA é desaconselhado um consumo de café superior a três chávenas por dia.
