Está também associada ao aumento da incidênciade doenças autoimunes e malignidades. Este síndrome está incluído nas doenças raras e temorigem numa grande variedade de mutações no gene que codifica para a proteína do Síndromede Wiskott-Aldrich (WASp), conduzindo a alterações na sua expressão e funções nas célulasdo sistema imunológico.WASp é uma proteína citoplasmática expressa, exclusivamente, em célulashematopoiéticas. Esta proteína desempenha uma função essencial na regulação dapolimerização da actina, que é a chave para a reorganização do citoesqueleto, desempenhandoum papel fundamental no controlo da morfologia e da motilidade celular. A ausência de WASpfuncional resulta num variadíssimo leque de defeitos funcionais da resposta da imunidade inatae adaptativa, como a migração celular, adesão, proliferação, fagocitose de patogénios invasorese formação de sinapses imunológicas.Nesta patologia, WAS, estão alteradas diversas funções do sistema imunitário,especificamente as células da linhagem mielóide, células dendríticas, células NK e os váriostipos de linfócitos. Se a origem biológica deste síndrome não for precocemente identificado etratado, a situação clínica pode evoluir para quadros manifestamente graves e conduzir àmorte do doente.O transplante de células estaminais hematopoiéticas é o tratamento de escolha parafenótipos mais graves do síndrome de Wiskott-Aldrich, sendo que nos fenótipos menosseveros tende-se a optar pelo tratamento das manifestações clínicas. Mais recentemente, aterapia génica tornou-se uma opção alternativa, embora ainda se encontre em investigação.Wiskott-Aldrich syndrome (WAS), is characterized by an immunodeficiency associatedwith a distinct clinical picture, which consists mainly in the appearance of eczema,microthrombocytopenia and immunodeficiency with recurrent infections. Also, it’s associatedto an increased incidence of autoimmune diseases and comorbidities. This syndrome isincluded in the class of rare diseases and has its origin in a wide variety of mutations in thegene that codes the Wiskott-Aldrich syndrome (WASp) protein, leading to changes in itsexpression and function towards the cells of the immune system.WASp is a cytoplasmic protein expressed exclusively in hematopoietic cells. Thisspecific protein plays an essential role in the regulation of actin polymerization, which is thekey to the reorganization of the cytoskeleton that plays an underlying role in the control ofcell morphology and motility. The absence of functional WASp results in a wide range offunctional defects, relatable to the innate and adaptive immunity response, such as: cellmigration, adhesion, proliferation, phagocytosis associated with invading pathogens andformation of immune synapses.In this pathology, WAS, several functions of the immune system are altered, specificallythe cells of the myeloid lineage, dendritic cells, NK cells and the various types of lymphocytes.If the biological origin of this syndrome is not identified and treated early, the clinical situationcan evolve to manifestly severe conditions and lead to the patient's death.Hematopoietic stem cell transplantation is the treatment of choice for the most severephenotypes of the Wiskott-Aldrich syndrome, with less severe phenotypes tending to treatclinical manifestations.
