Autonomous Nursing Interventions as Quality Sensitive Indicators for patient care with Noninvasive Ventilation

Resumo: | Introduction: Autonomous Nursing Interventions (IAN) may acquire the status of Quality Sensitive Indicators (ISQ) for critical patient care with Noninvasive Mechanical Ventilation (NIV), if their consistency of application is in accordance with the effectiveness of recovery. Objective/s: Recognizing the IAN of nurses in the care of critically ill patients with NIV, which, due to consistency of execution, can be considered ISQ to health care. Methods: This is a cross-sectional, exploratory and quantitative study, based on the application of a questionnaire, elaborated for this purpose, in a sample of 76 nurses, mostly female (82.9%), and age group between 36 and 45 years (51.3%), from the North, in Emergency and Intensive Care Services. Results: The eight IAN identified as ISQ were "(...) the register of the state of consciousness"; "(..) recording of hemodynamic parameters"; "use of antibacterial filter and humidification (...)"; "the measurement of the size of the interface is relevant"; "patient positioned in bed with headboard >30º"; "the interface is selected in order to suit the patient"; "the lift to the chair is indicated" and "application of vaseline on the lips and humidification of the oral cavity (...)". Conclusion: It reaffirms the pertinence of the nurses' autonomous competencies, in the provision and visibility of the provision of differentiated care, as well as the need to create a protocol for the use of NIV therapy for the critically ill. |

Objetivo/s: Reconocer las IAE de las enfermeras en la atención de pacientes críticamente enfermos con VNI, que, debido a la consistencia de la ejecución, puede considerarse ISC para la atención de la salud. Métodos: Se trata de un estudio transversal, exploratorio y cuantitativo, basado en la aplicación de un cuestionario, en una muestra de 76 enfermeros, en su mayoría mujeres (82,9%), y grupo de edad entre 36 y 45 años (51,3%), del Norte, en Servicios de Urgencias e Intensivos. Resultados: Las ocho IAE identificadas como ISQ fueron "(...) el registro del estado de conciencia"; "(..) registro de parámetros hemodinámicos"; "uso de filtro antibacteriano y humidificación (...)"; "la medición del tamaño de la interfaz es pertinente"; "paciente colocado en cama con cabecero >30º"; "la interfaz se selecciona para adaptarse al paciente"; "el levantamiento a la silla está indicado" y "aplicación de vaselina en los labios y humidificación de la cavidad oral (...)". Conclusión: Reafirma la pertinencia de las competencias autónomas de los enfermeros, en prestación de cuidados diferenciados, así como la necesidad de crear un protocolo para el uso de la terapia NVI para enfermos críticos.Introdução: As Intervenções Autónomas de Enfermagem (IAE) podem adquirir o estatuto de Indicadores Sensíveis de Qualidade (ISQ) aos cuidados ao doente crítico com Ventilação Mecânica Não Invasiva (VMNI), se a sua consistência de aplicação, estiver de acordo com a eficácia da recuperação. Objetivo/s: Reconhecer as IAE dos enfermeiros, no cuidado ao doente crítico com VMNI, que pela consistência de execução, podem ser consideradas ISQ aos cuidados de saúde. Métodos: Estudo transversal, exploratório e quantitativo, a partir da aplicação de um questionário, elaborado para o efeito, numa amostra de 76 enfermeiros, maioritariamente do sexo feminino (82,9%), e escalão etário entre 36 a 45 anos (51,3%), do Norte, em Serviços de Urgência e Medicina Intensiva. Resultados: As oito IAE identificadas como ISQ foram “(…) o registo do estado de consciência”; “(..) registo de parâmetros hemodinâmicos”; “utilização de filtro antibacteriano e humidificação (…)”; “a aferição do tamanho da interface é pertinente”; “doente posicionado no leito com cabeceira >30º”; “a interface é selecionada de forma a adequar-se ao doente”; “é indicado o levante para o cadeirão” e “aplicação de vaselina nos lábios e a humidificação da cavidade oral (…)”.
