A Organização Mundial da Saúde (OMS) estima que quase 70% dos doentes que sofrem de epilepsia poderiam viver sem crises se devidamente diagnosticados e tratados de forma adequada. Uma abordagem cirúrgica só é usada quando a estratégia farmacológica falha, também conhecida como epilepsia refratária à medicação antiepilética, e quando existe condições para a realização do procedimento. A ressonância magnética (RM) desempenha um papel central na avaliação e tratamento cirúrgico destes doentes com epilepsia refratária. Nestes casos, o sucesso da cirurgia da epilepsia depende fortemente da identificação de precisa de uma lesão epileptogénica em RM, e essa identificação é o parâmetro de prognóstico mais importante para estes doentes. Por isso, é de extrema importância que os exames de ressonância magnética estrutural sejam adquiridos de forma otimizada, utilizando um protocolo específico, e devem ser lidos por um especialista dedicado e com experiência na área, tendo em consideração todos os dados clínicos disponíveis. A existência de centros de referência de cirurgia de epilepsia, dotados de todas as valências especializadas para a avaliação e tratamento destes doentes, é fundamental para permitir o grau de experiência necessário para a obtenção dos melhores resultados. Neste trabalho iremos discutir os fundamentos do protocolo de RM estrutural da epilepsia e das sequências de RM nele usadas; rever as características imagiológicas das lesões epileptogénicas mais comuns; e analisar a casuística cirúrgica do Centro de Referência de Cirurgia de Epilepsia do Hospital Santa Maria/Centro Hospitalar Lisboa Norte no período de 1992 a 2021, avaliando a correlação entre o diagnóstico imagiológico e histopatológico.Epilepsy is a chronic noncommunicable disorder of the brain affecting around 50 million people worldwide. The World Health Organization (WHO) has estimated that nearly 70% of patients suffering from epilepsy could live seizure-free if properly diagnosed and adequately treated. A surgical approach is only used when the pharmacological strategy has failed, otherwise known as medically refractory epilepsy (MRE), and the required prerequisites are met. Magnetic resonance imaging (MRI) plays a central role in the management and evaluation of patients with refractory epilepsy. In these cases, the success of epilepsy surgery depends heavily on the accurate identification of an epileptogenic lesion on MRI, and this identification is the most important prognostic factor for these patients. Therefore, it is extremely important that structural MRI scans are optimally acquired, using a specific protocol, and should be read by a dedicated specialist with experience in the field, considering all available clinical data. The existence of centres of excellence in epilepsy surgery, equipped with all the specialized skills for the evaluation and treatment of these patients, is essential to allow for the degree of experience necessary to obtain the best results.
