No entanto, graças aos avanços nas áreas de terapêutica e prevenção, assistimos a uma visível diminuição da mortalidade infantil, sobretudo nos países desenvolvidos, não observando o mesmo sucesso na sobrevivência na idade adulta. Atualmente, a única terapêutica curativa com impacto na sobrevida do paciente é o Transplante de Células Hematopoiéticas (TCH). Embora já exista muita consistência bibliográfica nas indicações e metodologias, o TCH continua a ser uma opção sem total certeza acerca da segurança e outcomes, sendo ainda alvo de diversos estudos médico-científicos. Deste modo, foi realizado um estudo retrospetivo descritivo, com caracterização das crianças com drepanocitose seguidas no Hospital de Santa Maria (HSM) e identificação dos doentes com critérios para TCH. Resultados: De uma amostra inicial de 119 pacientes, 80 foram selecionados para serem analisados e caracterizados individualmente. Dezanove doentes (24%) foram considerados elegíveis para TCH, tendo como indicações mais comuns a lesão de órgão-alvo ou alterações cerebrovasculares na ressonância magnética. Apenas duas crianças das elegidas tinham já indicação para TCH. Conclusão: Apesar de se tratar de um estudo retrospetivo, os autores reforçam a importância de olhar para as crianças com drepanocitose como potenciais candidatas a uma cura, considerando a sua situação clínica e socioeconómica. Embora ainda haja um longo caminho a percorrer na implementação deste procedimento, os resultados internacionais são promissores, convocando a comunidade médica a rever as indicações do TCH e a definir a melhor abordagem para cada paciente.Sickle Cell Disease is nowadays one of the most common paediatric haematological diseases, with a great impact on quality of life, morbidity and infant mortality. However, thanks to advances in the areas of therapy and prevention, we are witnessing a visible decrease in infant mortality, especially in developed countries, with survival in adulthood not having the same success. Currently, the only curative therapy with an impact on patient survival is Hematopoietic Stem Cell Transplantation (HSCT). Although there is already a lot of bibliographic consistency in the indications and methodologies, HSCT remains an option without complete certainty about safety and outcomes, and is still the subject of several medical-scientific studies. Considering that, a descriptive retrospective study was carried out, with characterization of children with sickle cell disease followed up at Hospital de Santa Maria (HSM) and identification of patients with criteria for HSCT. Results: From an initial sample of 119 patients, 80 were selected to be analysed and characterized individually. Nineteen patients (24%) were considered eligible for HSCT, with the most common indications being end-organ damage or cerebrovascular alterations on MRI. Only two of the selected children already had an indication for TCH. Conclusion: Although this is a retrospective study, the authors reinforce the importance of looking at children with sickle cell disease as potential candidates for a cure, considering their clinical, social and financial status.
