Profile of patients undergoing treatment for tuberculosis in a health unit with a Family Health Care Team

Resumo: Introduction: Brasil is 14th amongst the 20 countries with 80% of tuberculosis cases (TB) in the world, with annual incidence of 46 new cases/100,000 inhabitants. The Health Unit of Divina Providencia Hospital, of SSC/ GHC, is a reference for TB treatment in an area of about 6,000 inhabitants. It’s expected about 6 new cases a year in the population covered by the hospital, considered a historic series and incidence in Porto Alegre, which currently is of 100/100,000. Objective: To describe the profile of TB carriers from USDP, with data on incidence, type of TB, serology for HIV, age, sex and start of treatment. Methods: Review of the registered cases in the USDP archives and review of the patient charts. Results: Between February 2007 and February 2008, 14 new cases of TB were registered in USDP. The ages were among 13 to 67 years, 8 men and 6 women. The average age of men was 38.6 years old and of women was 35.6 years old. Five were HIV positive and one person refused to do the HIV exam. Ten cases were exclusively of pulmonary TB and four cases of extra-pulmonary TB. The 14 cases were treated with E1 (RHZ). Out of the patients treated, 6 were discharged by cure (42%), 1 from abandonment (7%) and 1 death (7%), and 6 were still undergoing treatment by the end of the data collection (46.6%). Conclusion: the high incidence of TB, more than double than expected, in the USDP territory, indicates that these ratios need to be reviewed and compared to other Health Units that act in the same manner. The gathering of new cases through the search of respiratory symptomatics, as well as the vigilance of cases in treatment, is allowing for good detection and cure ratios, with good assent to treatment.

Son esperados cerca de 6 casos nuevos/año en una población cubierta por la USDP, considerando la serie histórica y la incidencia en Porto Alegre, que hoy es de 100/100.000. Objetivo: describir el perfil de los portadores de TB de la USDP, con datos sobre incidencia, forma de TB, serología para el sida, edad, sexo y adhesión al tratamiento. Métodos: Revisión de los registros, de los casos en los periódicos de la USDP e revisión de prontuarios. Resultados: Entre febrero de 2007 y febrero de 2008, se certificaron 14 nuevos casos de TB en USDP. Las edades se situaron entre 13 y 67 años, 8 hombres y 6 mujeres. La media de edad de los hombres han sido de 38-6 años y de las mujeres han sido 35,6.Cinco eran positivas para HIV y una persona se negó a realizar el examen. Las formas fueron: 10 casos exclusivos de TB pulmonar y 4 casos de TB extra pulmonar. Los 14 casos fueron tratados con E1(RHZ).Entre los pacientes tratados, 6 tuvieron liberación por cura (42%) , 1 por abandono (7%) y 1 óbito (7%), y 6 mantuvieron en tratamiento en el final de la colecta de datos (46,6%). Conclusión: La alta incidencia de TB, mucho más que era esperada, en territorio de USDP, indica que estas tasas necesitan ser revistas y comparadas a las de otras unidades de salud que actúan de la misma forma. Objetivo: Descrever o perfil dos portadores de tuberculose da USDP, com dados sobre incidência, forma de tuberculose, sorologia para o HIV, idade, sexo e adesão ao tratamento. Métodos: Revisão dos registros dos casos nos impressos da USDP e dos prontuários. Resultados: Entre fevereiro de 2007 e fevereiro de 2008, registraram-se 14 novos casos de tuberculose na USDP. As idades situaramse entre 13 e 67 anos, oito homens e seis mulheres. A média de idade dos homens foi 38,6 anos e das mulheres, 35,6. Cinco eram positivos para HIV e uma pessoa negou-se a realizar o exame. As formas foram: dez casos exclusivos de tuberculose pulmonar e quatro casos de tuberculose extrapulmonar. Os 14 casos foram tratados com E1 (RHZ). Dos pacientes tratados, seis tiveram alta por cura (42%), um por abandono (7%) e um óbito (7%), e seis permaneciam em tratamento no final da coleta de dados (46,6%). Conclusão: A alta incidência de tuberculose, mais do que o dobro da esperada, no território da USDP, indica que estas taxas necessitam ser revistas e comparadas às de outras Unidades de Saúde que atuam da mesma forma.
