Resumo: | Objective: To analyze the determinants of maternal and newborn health associated with exclusive breastfeeding in three maternity hospitals in Espírito Santo. Method: Cross-sectional, multicenter study, with data from a larger study called Projeto Viver. The sample consisted of 3,438 mother-infant pairs. The relationship between exclusive breastfeeding practiced in maternity hospitals, data related to the mother, newborn and prenatal care was performed using the chi-square test. Logistic regression was used to assess the variables that most influence women in exclusive breastfeeding. The significance level adopted was 5%. Results: Exclusive breastfeeding before hospital discharge was 85.2%. Breastfeeding in the first hour of life was not practiced by 91.2% and 75.4% of women did not participate in educational activities during prenatal care. In logistic regression, women under 19 years of age and babies born with normal weight and at term are more likely to practice exclusive breastfeeding while still in the maternity ward. Conclusion: The fulfillment of public policies in favor of breastfeeding must be mirrored in the need to guarantee economic and social support so that exclusive breastfeeding reaches the goals established by the WHO. |

Método: Estudio transversal, multicéntrico, con datos de un estudio mayor denominado Projeto Viver. La muestra estuvo compuesta por 3.438 parejas madre-hijo. La relación entre la lactancia materna exclusiva practicada en las maternidades, los datos relacionados con la madre, el recién nacido y el control prenatal se realizó mediante la prueba de chi-cuadrado. Se utilizó regresión logística para evaluar las variables que más influyen en las mujeres en la lactancia materna exclusiva. El nivel de significación adoptado fue del 5%. Resultados: La lactancia materna exclusiva antes del alta hospitalaria fue del 85,2%. La lactancia materna en la primera hora de vida no fue practicada por el 91,2% y el 75,4% de las mujeres no participó de actividades educativas durante el prenatal. En la regresión logística, las mujeres menores de 19 años y los bebés nacidos con peso normal ya término tienen más probabilidades de practicar la lactancia materna exclusiva mientras aún están en la sala de maternidad. Conclusiones: El cumplimiento de las políticas públicas a favor de la lactancia materna debe reflejarse en la necesidad de garantizar el apoyo económico y social para que la lactancia materna exclusiva alcance las metas establecidas por la OMS.Objetivo: Analisar os determinantes em saúde maternos e de recém-nascidos associados ao aleitamento materno exclusivo em três maternidades do Espirito Santo. A relação entre o aleitamento materno exclusivo praticado nas maternidades, dados relacionados à mãe, recém-nascido e pré-natal foi realizada através do teste qui-quadrado. Regressão logística foi utilizada para avaliar as variáveis que mais influenciam as mulheres na amamentação exclusiva. O nível de significância adotado foi de 5%. Resultados: O aleitamento materno exclusivo antes da alta hospitalar foi de 85,2%. O aleitamento materno na primeira hora de vida não foi praticado por 91,2% e 75,4% das mulheres não participaram de atividade educativa durante o pré-natal. À regressão logística, mulheres menores de 19 anos e bebês nascidos com peso normal e a termo apresentam maiores chances de praticar a amamentação exclusiva ainda na maternidade.
