DIAGNOSTIC INDICATORS FOR PRESSURE INJURY RISK IN ORTHOPEDIC TRAUMA PATIENTS

Resumo: | Objective: to analyze the prevalence of the nursing diagnosis risk of pressure injury and its respective indicators in patients with orthopedic trauma. Method: This is a cross-sectional, descriptive study with a quantitative approach, carried out with patients in the pre and post-operative period of orthopedic surgeries, in two different scenarios: in the female and male ward of a referral hospital for trauma-orthopedics, located in the interior of Ceará, Brazil. The collected data were compiled in Excel software, analyzed in R software version 3.4.4 and organized into tables. Result: 93 patients were evaluated, predominantly female, mixed race, with a mean age of 46.29 years, who did not have a partner, with an average family income greater than one minimum wage and with low education. There was a prevalence in the preoperative period, in the majority, with involvement of upper limbs, using orthopedic immobilization, with clinically stable parameters (vital signs). The prevalence of the diagnosis of Risk of Pressure Injury was 98.92%, with a high mean number of risk factors per patient (10.31). The most prevalent risk factors were surface friction (78.49%) and reduced mobility (78.49%), followed by shear forces (73.12%). It is evident that there is a strong indication that patients admitted to an orthopedic clinic are at risk of developing PI. Conclusion: The identification of the causal factors of pressure injuries guarantees the nurse the planning and elaboration of the care plan with specific interventions, in order to guarantee the patient's safety, decrease of injuries and better quality of life. |

Método: Se trata de un estudio transversal, descriptivo, con abordaje cuantitativo, realizado con pacientes en pre y postoperatorio de cirugías ortopédicas, en 2 escenarios diferentes: en sala femenina y masculina de un hospital de referencia de traumatología. ortopedia, ubicada en el interior de Ceará. Los datos recolectados fueron compilados en el software Excel, analizados en software R versión 3.4.4 y organizados en tablas. Resultados: Se evaluaron 93 pacientes, predominantemente del sexo femenino, mestizos, con una edad media de 46,29 años, sin pareja, con renta familiar media superior a un salario mínimo y con baja escolaridad. Hubo un predominio en preoperatorio, en su mayoría, con compromiso de miembros superiores, utilizando inmovilización ortopédica, con parámetros clínicamente estables (signos vitales). La prevalencia del diagnóstico de Riesgo de Lesión por Presión fue del 98,92%, con una media alta de factores de riesgo por paciente (10,31). Los factores de riesgo más prevalentes fueron el rozamiento superficial (78,49 %) y la movilidad reducida (78,49 %), seguidos de fuerzas de cizallamiento (73,12 %). Es evidente que existe un fuerte indicio que los pacientes ingresados en una clínica ortopédica corren el riesgo de desarrollar IP. Conclusión: La identificación de factores causales de lesiones por presión garantiza al enfermero la planificación y elaboración del plan de cuidados con intervenciones específicas, con el fin de garantizar la seguridad del paciente, disminución de lesiones y mejor calidad de vida.Objetivo: analisar a prevalência do diagnóstico de enfermagem risco de lesão por pressão e dos seus respectivos indicadores em pacientes com trauma ortopédico. Resultados: Foram avaliados 93 pacientes, predominantemente do sexo feminino, cor parda, com média de idade de 46,29 anos, que não obtinham companheiro (a), com renda familiar média maior que um salário mínimo e de baixa escolaridade. Evidenciou-se prevalência no período pré-operatório, em maioria, com acometimento de membros superiores, em uso de imobilização ortopédica, com parâmetros (sinais vitais) clinicamente estáveis. A prevalência do diagnóstico de Risco de lesão por pressão foi de 98,92%, com elevada média de número de fatores de risco por paciente (10,31). Os fatores de risco mais prevalentes foram atrito em superfície (78,49%) e redução da mobilidade (78,49%), seguido de forças de cisalhamento (73,12%). Evidencia-se, que, há um forte indicativo que pacientes internados em clínica ortopédica possuem o risco de desenvolvimento de LP.
