Avaliação de critérios de sucesso em um programa multiprofissional de tratamento da obesidade em adolescentes : análise em curto e longo prazo

Resumo: Evaluate criteria of success in a multidisciplinary program of obesity treatment: analysis in short- and long-term. It was included 22 adolescents with excesso body weight aged between 10 and 18 years. It was realized anthropometric assessments, body composition, physical fitness, resting herat rate, and metabolic parameters. It was assessed pubertal development and stage of readiness to change eating and exercise behavior using the questionnaire ¨Stage of Change¨ (SOC), besides, it was assessed health-related quality of life using the generic questionnaire PedsQLTM 4.0. It was assessed after the 16-week intervention the success rate attained according to the criteria/cut off points proposed by Nardo Junior et al. following four possibilities of determination of success in separated groups (success vs not success). The rate of success was associated with sex, chronological and biological age and stage of readiness to change eating and exercise behavior. The association was verified by 2x2 Chi-square and Chi-square of tendency. After that, it was compared the results after three semesters of intervention for adolescentes who obtained and those who did not obtained success after 16 weeks of intervention. It was used the Analysis of Variance (ANOVA) for repeated measures and mixed ANOVA for repeated measures for the comparisons between moments. When diferences in baseline were observed, this moment was used as covariate and the behavioral factors associated with success in 16 weeks of intervention observed in study 1 were also used as covariates. Significance was set at p<0.05. It was also applied the analysis of magnitude of changes. Three semesters of intervention were capable to promote improvements in BMI, BMI Z score, flexibility, abdominal strength/resistance, handgrip strength, cardiorespiratory fitness, absolute and relative body fat, WC, DBP, LDL-c, HDL-c, VLDL-c, TG, RFMS and physical domain of HRQoL, besides, the criteria EsSu-ACCO and EsSu-GORD were associated with greater stages of readiness to change behaviors related to the amount of fat on diet, consumption of fruits and vegetables, increase in physical activity and the combination of all domains of SOC questionnaire (p<0.05). With respect to the success attained when the EsSu-PMTO was considered, no associations were observed, however, sex and chronological age presented associations close to significance (p <0.10) with this criterion. Independently of of the criteria used, the group who obtained success presented improvements in more parameters than the group who did not obtain success. The parameters that improved more in the success group compared to the non-success group were the risk factors for metabolic syndrome, and the analysis in magnitudes of changes confirmed that, considering that the non-success group presented more 'unclear' results. The present study observed that three semesters of multidisciplinar intervention were benefical and promoted changes in body composition, physical fitness, metabolic parameters and HRQoL, moreover, attain the success in 16 weeks of intervention may be na indicative of better results in long term. The success obtained in the criteria EsSu-ACCO and EsSu-GORD were associated with greater stages of change. No association was observed for the criterionEsSu-PMTO.

Foram incluídos no estudo 22 adolescentes com excesso de peso e idades entre 10 e 18 anos. Foram realizadas avaliações antropométricas, composição corporal, aptidão física, frequência cardíaca de repouso e parâmetros bioquímicos. Foi avaliado o desenvolvimento puberal e o estágio de prontidão para mudança no comportamento usando o questionário Stage of Change (SOC), além disso, foi avaliado a qualidade de vida relacionada à saúde usando o questionário genérico PedsQLTM 4.0. Foram avaliadas, após 16 semanas de intervenção, a taxa de sucesso atingida de acordo com os critérios/pontos de corte propostos por Nardo Junior et al. seguindo quatro possibilidades de determinação do sucesso e separados em dois grupos (sucesso vs não sucesso). A taxa de sucesso foi associada ao sexo, idade biológica, idade cronológica e estágio de prontidão para mudança no comportamento alimentar e atividade física. A associação foi verificada pelo teste Qui-quadrado 2x2 e Qui-quadrado de tendência. Após isso foi comparada com os resultados após três semestres de intervenção para os adolescentes que obtiveram e os que não obtiveram sucesso em 16 semanas. Foi utilizado o teste Analysis of Variance (ANOVA) de medidas repetidas e a ANOVA mista de medidas repetidas para comparação entre os três momentos do estudo. Quando obsevou-se diferenças no baseline, foi usado esse momento como covariável e os fatores comportamentais associados com o sucesso em 16 semanas observados no estudo 1 foram também usados como covariáveis. A significância foi pré-estabelecida em p<0,05. Foi aplicada a análise de inferências práticas baseadas em magnitudes. Três semestres de intervenção são capazes de promover melhoras sobre IMC, escore Z do IMC, flexibilidade, força/resistência abdominal, força de preensão manual, aptidão cardiorrespiratória, gordura corporal absoluta e relativa, massa magra, CC, PAD, LDL-c, HDL-c, VLDL-c, TG, FRSM e domínio físico da QVRS, além disso, os critérios EsSu-ACCO (Escore de sucesso critério antropometria e composição corporal) e EsSu-GORD (Escore de sucesso critério gordura corporal) foram associados a estágios mais avançados de prontidão para mudança do comportamento em relação à quantidade de gordura da dieta, consumo de frutas e vegetais, aumento da atividade física e a combinação de todos os domínios do questionário SOC (p<0,05). No que diz respeito ao sucesso atingido quando levado em consideração o critério EsSu-PMTO (Escore de sucesso critério global), nenhuma associação significativa foi observada, no entanto, o sexo e a idade cronológica apresentaram associações próximas do nível de significância (p <0,10) com este critério de sucesso. Independente do critério de sucesso em 16 semanas aplicado, o grupo que obteve sucesso apresentou melhoras em um número maior de parâmetros em comparação com o grupo que não apresentou sucesso. Os parâmetros que mais melhoraram no grupo que obteve sucesso em comparação ao grupo que não obteve foram os fatores de risco para síndrome metabólica, e a análise de inferências práticas confirmou esses achados, tendo em vista que nos grupos sem sucesso o número de parâmetros inconclusivos foi maior. Nosso estudo observou que três semestres de intervenção multiprofissional foram benéficas para mudanças na composição corporal, aptidão física, parâmetros metabólicos e qualidade de vida relacionada à saúde e que atingir o sucesso em 16 semanas de intervenção pode ser um indicativo de melhores resultados em longo prazo de intervenção multiprofissional. O sucesso atingido nos critérios EsSu-ACCO e EsSu-GORD se associaram aos estágios mais avançados de prontidão para mudança do comportamento.
