Resumo: Inflammatory Bowel Disease (IBD) and periodontitis are characterized by a complex etiopathogenesis, late diagnosis and for causing a worsening in the quality of life of individuals, so the identification of reliable biomarkers for an early and accurate diagnosis is an essential point for the correct management of these diseases. conditions. The aim of this study was to evaluate salivary metabolome in IBD patients with periodontitis and healthy patients without periodontitis. For each of the study participants, a medical record containing personal data, anamnesis, data related to the gastrointestinal system and dental data (visible plaque index, and periogram composed of pocket depth on probing, clinical attachment loss and bleeding on probing) was filled out. . Forty-nine patients were selected, divided into four groups, seven with Crohn's Disease (CD) and periodontitis, five with Idiopathic Ulcerative Colitis (IUGR) and periodontitis, twenty-three systemically healthy patients with periodontitis and fourteen systemically healthy patients without periodontitis. The samples consisted of 4mL of unstimulated saliva. For each patient, 1mL of saliva was added to 20μl of protease inhibitor, centrifuged for 8 minutes at 8000 revolutions per minute and at laboratory temperature. Then, the saliva supernatant was transferred to a new Eppendorf and frozen at -80°C until the samples were processed. Afterwards, the samples were submitted to analysis in a 500 MHz Bruker nuclear magnetic resonance spectrometer, the spectra obtained were submitted to multivariate analysis using PLS-DA, sPLS-DA and univariate analysis using chi-square and t tests (SPSS 20.0, IL, USA), with a significance level of p < 0.05. Thirteen metabolites were identified in the saliva samples (butyrate, caproate, ethanol, glucose, sucrose, glutamine, glutamate, hydroxybutyrate, threonine, lysine, valine and N-acetyl group). Patients with CD and periodontitis had statistically significant levels of butyrate, caproate, ethanol and hydroxybutyrate when associated with controls with periodontitis. Between patients with UC and periodontitis and controls with periodontitis, there was a statistically significant difference in the levels of butyrate, caproate, sugar region, threonine and valine. Among controls with and without periodontitis, isoleucine levels were statistically significant. The analysis of saliva by NMR was able to reveal significant differences in the metabolic profile between the four groups analyzed, showing that there may be a relationship, at the metabolic level, between IBD and periodontitis. However, more studies are needed and it is important that there is methodological standardization in future studies.

O objetivo deste estudo foi avaliar o metaboloma salivar em pacientes acometidos por DII com periodontite e pacientes saudáveis sem periodontite. Para cada um dos participantes do estudo um prontuário contendo dados pessoais, anamnese, dados relacionados ao sistema gastrointestinal e dados odontológicos (índice de placa visível, e periograma composto por profundidade de bolsa a sondagem, perda de inserção clínica e sangramento a sondagem) foi preenchido. Foram selecionados 49 pacientes, divididos em quatro grupos, sendo sete com Doença de Crohn (DC) e periodontite, cinco com Retocolite Ulcerativa Idiopática (RCUI) e periodontite, vinte e três pacientes sistemicamente saudáveis com periodontite e quatorze pacientes sistemicamente saudáveis sem periodontite. As amostras foram constituídas de 4mL de saliva não estimulada. Para cada pacientes, 1mL de saliva foi adicionado a 20μl de inibidor de protease, centrifugado durante 8 minutos, a 8000 rotações por minuto e à temperatura do laboratório. Em seguida, o sobrenadante da saliva foi transferido para um novo Eppendorf e congelado a -80°C até o processamento das amostras. Após, as amostras foram submetidas à análise em um espectrômetro de ressonância magnética nuclear Bruker de 500 MHz, os espectros obtidos foram submetidos à análise multivariada usando PLS-DA, sPLS-DA e análise univariada por meio de chi -quadrado e testes t (SPSS 20.0, IL, EUA), com nível de significância de p < 0,05. Treze metabólitos foram identificados nas amostras de saliva (butirato, caproato, etanol, glicose, sacarose, glutamina, glutamato, hidroxibutirato, treonina, lisina, valina e grupo N-acetil). Pacientes com DC e periodontite apresentaram níveis estatisticamente significantes de butirato, caproato, etanol e hidroxibutirato quando associados aos controles com periodontite. Entre os pacientes com RCUI e periodontite e os controles com periodontite houve diferenã estatisticamente significante nos níveis de butirato, caproato, região de açúcar, treonina e valina. Entre os controles com e sem periodontite os níveis de isoleucina foram estatisticamente significantes. A análise da saliva por RMN foi capaz de revelar diferenças significativas no perfil metabólico entre os quatro grupos analisados, mostrando que pode haver uma relação, a nível metabólico, entre DII e periodontite.
