Resumo: Fibromyalgia (FM) is a rheumatic syndrome characterized by chronic and diffuse musculoskeletal pain. It is an affection of a class of disorders that presents diagnostic and therapeutic challenges to Western medical rationality, mainly due to chronic pain. The processes of social interaction, in the family, medical and work spheres, promote embarrassing and illegitimate relationships, with a lack of recognition Patients is not interpreted under the assumption of the total social fact. Studies highlight the role of interdisciplinary treatment (IT) and corporal practices of health (CPH) in the treatment of the syndrome. CPH comprise the human movement endowed with social relationships of health care and reception, while IT allows a better understanding of patients in social, psychological and physical terms. The theory of gift, based on the acts of giving, receiving and giving back care goods, provides recognition and legitimacy to the health-disease process of FM, in line with the assumption of the totality of the social fact. The study's thesis supports IT and PCS as circuits of legitimacy and recognition, where gifts of health care are exchanged between health professionals and patients with FM. The health professional offers health care in the treatment of the patient, who, in turn, receives this gift and reciprocates by continuing the circuit. The objective is to understand the IT and CPH for patients with FM as a system for the exchange of gifts health care that is established between patients and health professionals. This is a qualitative study carried out in the Project “Traramento Interdisciplinar para Pacientes com Fibromialgia” (TIF) through the application of an open interview and field observation with 8 patients diagnosed with FM and a semi-structured questionnaire with 9 health professionals. Participated in the study the patients who entered the TIF in 2019 and the health professionals involved with TIF. The categorical Content Analysis constructed three thematic categories: "Interdisciplinary treatment for patients with fibromyalgia: care practices and reception", "Constitution of the donation in the health care process" and "Body, health and chronic pain from the insertion in interdisciplinary treatment". The TIF was structured as a space for health care practices and reception, in terms of territorialization, decentralization and health care. The constitution of the gift in the health care process occurred through a shift to the other, symbolic actions in the relational circuit, gift of hospitality, gift of recognition, gift of conversation and hermeneutic cycle. The gift reciprocity circuit promoted new constructions, techniques and social uses of the body, health and chronic pain when entering the TIF. The conclusion of the study points to IT and CPH as spaces developed in the meeting between patients and health professionals, from the cycle of the triple obligation to give, receive and repay gifts of health care, which resulted in healthier social uses and legitimate aspects of the body and pain, processes of social interaction in the context of the recognition of symbolic effectiveness and in the assumption of totality of the social fact in terms of the health-disease process of FM.

Trata-se de um acometimento de uma classe de desordens que apresenta desafios diagnósticos e terapêuticos à racionalidade médica ocidental, principalmente por conta da dor crônica. Os processos de interação social, nos âmbitos familiar, médico e laboral, promovem relações constrangedoras e de ilegitimidade, com ausência de reconhecimento. Os pacientes não são interpretados no pressuposto do fato social total. Estudos ressaltam o papel do tratamento interdisciplinar (TI) e das práticas corporais de saúde (PCS) no tratamento da síndrome. As PCS compreendem o movimento humano dotado de relações sociais de cuidado em saúde e acolhimento, enquanto o TI possibilita uma melhor compreensão dos pacientes em termos sociais, psicológicos e físicos. A teoria da dádiva, a partir dos atos de dar, receber e retribuir bens de cuidado, fornece reconhecimento e legitimidade ao processo saúde-doença da FM, alinhada ao pressuposto de totalidade do fato social. A Tese do estudo sustenta o TI e as PCS como circuitos de legitimidade e reconhecimento, onde ocorrem a troca de dádivas de cuidado em saúde, entre profissionais de saúde e pacientes com FM. O profissional de saúde oferece o cuidado em saúde no tratamento ao paciente, que, por sua vez, recepciona este dom e retribui dando continuidade ao circuito. O objetivo é compreender o TI e as PCS para pacientes com FM como um sistema de troca de dádivas de cuidado em saúde que se estabelece entre os pacientes e os profissionais de saúde. Trata-se de um estudo qualitativo realizado no Projeto “Tratamento Interdisciplinar para pacientes com fibromialgia” (TIF) mediante entrevistas abertas e observações em campo com 8 pacientes diagnosticadas com FM e questionário semiestruturado com 9 profissionais de saúde. Participaram do estudo as pacientes que ingressaram em 2019 no TIF e os profissionais de saúde envolvidos com o TIF. A Análise do Conteúdo temática construiu três categorias: “Tratamento interdisciplinar para pacientes com fibromialgia: práticas de cuidado e acolhimento”, “Constituição da dádiva no processo de cuidado em saúde” e “Corpo, saúde e dor crônica a partir da inserção no tratamento interdisciplinar”. O TIF estruturou-se enquanto espaço de práticas de cuidado em saúde e acolhimento, em termos de territorialização, descentralização e atenção à saúde. A constituição da dádiva no processo de cuidado em saúde ocorreu mediante um deslocamento ao próximo na trama relacional, dom da hospitalidade, dom do reconhecimento e dom da conversação. Novos usos sociais do corpo, da saúde e dor crônica na inserção no TIF foram proporcionados. The categorical Content Analysis constructed three thematic categories: "Interdisciplinary treatment for patients with fibromyalgia: care practices and reception", "Constitution of the donation in the health care process" and "Body, health and chronic pain from the insertion in interdisciplinary treatment". The TIF was structured as a space for health care practices and reception, in terms of territorialization, decentralization and health care. The constitution of the gift in the health care process occurred through a shift to the other, symbolic actions in the relational circuit, gift of hospitality, gift of recognition, gift of conversation and hermeneutic cycle. The gift reciprocity circuit promoted new constructions, techniques and social uses of the body, health and chronic pain when entering the TIF. The conclusion of the study points to IT and CPH as spaces developed in the meeting between patients and health professionals, from the cycle of the triple obligation to give, receive and repay gifts of health care, which resulted in healthier social uses and legitimate aspects of the body and pain, processes of social interaction in the context of the recognition of symbolic effectiveness and in the assumption of totality of the social fact in terms of the health-disease process of FM.
