Acolhimento, redes sociais e produção do cuidado na Atenção Básica

Resumo: | This study has as its object the welcoming, social networks and production of care in Primary Health Care (ABS). Objective: To analyze reception from the perspective of care production in a Primary Health Care unit in the city of Rio de Janeiro. Methodology: Descriptive study with a qualitative and quantitative approach outlined by the systematic observation of the structural, relational and prescriptive aspects of the field, and analysis from the reception of the actors' relations in a care production perspective. Data were analyzed through Social Network Analysis (ARS), using Ucinet and Netdraw software that allowed the construction of the sociogram and the generation of centrality measures of the actors, being discussed in light of Pierre Bourdieu's theory. Results: Looking at ABS allowed us to re-signify it as a field, and it can be observed that it is shaped by rules that guide it and give meaning to its existence. In addition to the norms that dictate how the field should work and how the actors should act, different ways of perceiving and using the host were observed, which did not emerge through systematic observation as a health surveillance strategy for care production. . The ARS allowed us to understand how the relationships between the actors conform through the reception. The position of proximity of medical professionals and nurses of other professionals registers greater prestige and authority of these professions, compared to other professions in the field. The nursing technicians for their responsibility regarding the technical activities and the important role in the reception, since all demands are directed to them, appear in the highlighted network. As for the exported metrics, the density one was low, which indicates that not all actors are in direct contact with each other, exchanging information or any kind of resources, making the network uncohesive. In turn, all actors with the highest degree of centrality, proximity and intermediation are of higher level, except for nursing technicians. The portrayal of the relationship of the actors from the reception reiterates the findings of systematic observation, elucidating a logic of care production, which focuses on punctual care and problem solving. Conclusion: The results obtained by systematic observation and ARS were confluent. The fact of being welcomed by the observed and portrayed in the sociogram is configured as an access regulator and support device for risk classification and meeting spontaneous demand, implies rethinking rules, processes and the actions of actors in the field. The latter is revealed in the attitude, attitude and decision-making of the actors that permeated by the habitus and with the different capitals they possess could make the reception a potent health surveillance strategy for care production. |

Objetivo: Analisar o acolhimento na perspectiva da produção do cuidado em uma unidade de Atenção Básica em Saúde do Município do Rio de Janeiro. Metodologia: Estudo descritivo, com abordagem qualitativa e quantitativa, delineado pela observação sistemática dos aspectos estruturais, relacionais e prescritivos do campo, e análise a partir do acolhimento das relações dos atores em uma perspectiva de produção do cuidado. Os dados foram analisados mediante Análise de Redes Sociais (ARS), com a utilização dos softwares Ucinet e Netdraw, que permitiram a construção do sociograma e a geração das medidas de centralidade dos atores, sendo discutidos à luz da teoria de Pierre Bourdieu. Resultados: O olhar sobre a ABS permitiu ressignificá-la como um campo, sendo possível observar que ela é conformada por regras que a orientam e dão sentido à sua existência. Observou-se, para além das normativas que ditam como o campo deve funcionar e como os atores devem agir, formas distintas de perceber e utilizar o acolhimento, em que esse não emergiu através da observação sistemática como estratégia de vigilância em saúde para produção do cuidado. A ARS permitiu compreender como as relações entre os atores se conformam através do acolhimento. A posição de proximidade dos profissionais médicos e enfermeiros dos demais profissionais registra maior prestígio e autoridade dessas profissões em relação às outras profissões do campo. Os técnicos de enfermagem, pela responsabilidade que detêm no que concerne às atividades de cunho técnico e pelo papel de relevo no acolhimento, já que todas as demandas são direcionadas para eles, aparecem na rede em destaque. Quanto às métricas exportadas, a de densidade foi baixa, o que indica que nem todos os atores estão em contato direto uns com os outros, trocando informação ou qualquer tipo de recurso, tornando a rede pouco coesa. Por sua vez, todos os atores com maior grau de centralidade, proximidade e intermediação são de nível superior, com exceção dos técnicos de enfermagem. O retrato da relação dos atores a partir do acolhimento reitera os achados da observação sistemática, elucidando uma lógica de produção do cuidado, cujo foco é o atendimento pontual e a resolução de problemas. Conclusão: Os resultados obtidos pela observação sistemática e ARS mostraram-se confluentes. O fato de o acolhimento pelo observado e pelo retratado no sociograma configurar-se como regulador do acesso e dispositivo de apoio para classificação de risco e atendimento à demanda espontânea implica repensar normativas, processos e o agir dos atores no campo.
