Resumo: | Hantaviruses are rodent-borne enveloped RNA-viruses belonging to Bunyaviridae family that have worldwide distribution and are shelter in Sigmodontinae subfamily in America. Hantavirus have been identified as etiologic agents of infection human, which can range from mild disease to severe disease, such as Hantavirus Cardiopulmonary Syndrome (HPCS). Human infection is acquired by inhanalation of aerosols containing excreta of rodents infected by hantavirus. However, there are few records in the Northeast. So far, in the Alagoas State, to date there is no reports of hantavirus infections, although with conditions favorable to circulation of hantavirus, for example: substitution of Atlantic Florets for sugar agro cane industry, in East Mesoregion; corn culture, in Agreste; and the dry climate in the Sertão Mesoregion. The aim this study it was to collect evidence of the occurrence of human hantavirus infections in different mesoregions Alagoas State. Sera of volunteers healthy were collected and used to detect IgG antibodies against a recombinant N protein of Araraquara hantavirus (rN ARAV), by enzyme immunoassay (ELISA), as well as epidemiological data were obtained by questionnaires. The samples were considerated positive when titers were equal or greater of 200. In this study was included 928 individuals, mean age 35 years (sd=16.62), with median of 32 years. From the East Mesoregion, 704 volunteers participated, of which 54 had anti-rN ARAV (prevalence of 7.67%). In Agreste, 155 volunteers participated, of these, three had these antibodies. In the Sertão Mesoregion, 69 participants were included, of which 2 were serum-reactive. The global prevalence was 6.36% (59/928), wich titers ranged from 200 to 3,200. Of soropositive volunteers, 72.35% of them attested that had never lived and 11.86% said that developed disease with respiratory failure. The human antigenic contact with hantavirus induces long term IgG antibodies, even that when the HPCS development were not occurred during the acute infection. The results of this study indicated the occurrence of local infections by hantavirus in Alagoas State, including in individuals without respiratory disease history in the past. Thus, taken together, these findings suggest the circulation of hantavirus in Alagoas State and the occurrence of silent infections, both inapparent or symptomatic infections but without diagnosis. This is the first study to demonstrate serological evidence of past human infections by hantavirus in individuals from three mesoregions of Alagoas State. Our findings provide new insights into the epidemiology of hantaviruses in the Northeast Region from Brazil. |

Possuem como material genético RNA e nas Américas são abrigados em roedores da subfamília Sigmodontidae, os quais se comportam como reservatórios. No ser humano, as formas de infecção por hantavírus incluem apresentações clínicas que variam de formas leves da doença até quadros mais graves, com o desenvolvimento da Síndrome Pulmonar Cardiovascular por Hantavírus (SPCVH). Os hantavírus são transmitidos por meio da inalação de aerossóis formados das excretas de roedores silvestres infectados. No Brasil, casos de SPCVH têm sido registrados em todo país, acometendo principalmente trabalhadores rurais. No entanto, são poucos os registros na Região Nordeste. Até o momento, o estado de Alagoas não possui registro de SPCVH, embora apresente condições favoráveis à circulação de hantavírus, como por exemplo: a substituição de Mata Atlântica pela cultura da cana-de-açúcar, na Mesorregião do Leste; o cultivo do milho, no Agreste e o clima seco presente na Mesorregião do Sertão alagoano. Assim, este estudo teve por objetivo levantar evidências da ocorrência de infecções humanas por hantavírus nas diferentes mesorregiões do estado de Alagoas. Para tanto, foi realizado inquérito sorológico em indivíduos saudáveis de três municípios das mesorregiões alagoanas: Coruripe, Mesorregião do Leste; Palmeira dos Índios, Agreste; e Olho D’Água do Casado, Sertão, nas quais foram coletadas amostras de soro dos participantes para a pesquisa de anticorpos IgG contra a proteína N recombinante do hantavírus Araraquara (rN ARAV) através do ensaio imunoenzimático (ELISA) e dados epidemiológicos foram obtidos por meio da aplicação de questionários. As amostras foram consideradas positivas quando obtiveram títulos iguais ou acima de 200. Neste estudo foi incluído um total de 928 indivíduos, com média de idade de 35 anos (sd=16,62), mediana de 32 anos. Da mesorregião do Leste, participaram 704 voluntários, dos quais 54 possuíam anticorpos anti-rN ARAV (prevalência de 7,67%). No Agreste, participaram 155 voluntários, destes 3 possuíam os anticorpos pesquisados (prevalência de 1,94%). No Sertão, foram incluídos 69 participantes, dos quais 2 foram sororreagentes (prevalência de 2,9%). No total, a prevalência foi de 6,36% (59/928), com títulos de 200 a 3.200. Entre os sororreagentes, 72,35% relataram nunca ter estado em outro estado e 11,86% afirmaram ter sofrido de doença com insuficiência respiratória. O contato antigênico do ser humano com hantavírus induz anticorpos IgG de longa duração, ainda que não venha a desenvolver a SPCVH durante a infecção aguda. Os achados deste estudo indicam a ocorrência de infecções autóctones por hantavírus em Alagoas, inclusive entre indivíduos com história de doença respiratória no passado. Deste modo, tomados em conjunto, estes achados sugerem a circulação de hantavírus no estado e a ocorrência de infecções silenciosas, com sintomatologia inaparente ou ainda sintomática, mas sem diagnóstico.
