Characterization of the physical exercise orientation for patients with diabetes and prediabetes assisted in a university hospital

Resumo: Introduction: The pillars of the treatment of Diabetes Mellitus (DM) are the association of drug treatment, the adoption of a healthy diet, and regular practice of physical exercises. Despite the various benefits described for patients with DM, regular physical exercise remains a challenge. Objective: To identify and characterize the physical exercise orientation delivered to patients with DM or prediabetes assisted in an outpatient clinic to treat or prevent DM. Material and Methods: Cross-sectional observational study. Between November 2018 and April 2019, data were collected by active search in the medical records of patients with type 1 DM (DM1), type 2 DM (DM2) or prediabetes assisted in a Multidisciplinary Diabetes Outpatient Clinic of the University Hospital of the Federal University of Juiz de Fora, including those diagnosed. Medical records with more than one attendance during the data collection period and from patients who did not have an attendance until the time of collection were excluded. Descriptive statistical analysis was performed by calculating simple frequencies and percentages for all variables and mean and standard deviation for numerical variables. Results: Of the 192 medical records, 169 were considered for analysis. Ninety-two percent of the sample was characterized with a diagnosis of DM2 (57 ± 11 years), and females were the most prevalent. Seventy-eight percent of the sample that contained information about physical activity in their medical records was sedentary. In 83% of the medical records, there was no documented physical exercise recommendation, and the 17% that presented the description were medical records of patients with DM2. The indications for “physical activity”, “lifestyle change” and “water aerobics” were the most documented terms for the recommendation. Conclusion: This study revealed the low registration rate on the orientation for physical exercise of patients with DM.

Apesar dos diversos benefícios descritos para pacientes com DM, a prática regular de exercício físico permanece sendo um desafio. Objetivo: Identificar e caracterizar a orientação para prática de exercícios físicos entregue à pacientes com DM ou pré-DM acompanhados em um serviço ambulatorial para tratamento ou prevenção do DM. Material e Métodos: Estudo observacional transversal. Entre novembro de 2018 e abril de 2019 realizou-se coleta de dados por busca ativa nos prontuários de pacientes em acompanhamento no Ambulatório Multidisciplinar de Diabetes do Hospital Universitário da Universidade Federal de Juiz de Fora, sendo incluídos neste estudo pacientes com diagnóstico de DM tipo 1 (DM1), tipo 2 (DM2) ou pré-DM. Foram excluídos prontuários de pacientes com mais de um atendimento no período da coleta de dados ou que não foram atendidos até o momento da coleta. Foi realizada análise estatística descritiva por meio do cálculo de frequências simples e percentuais para todas as variáveis e média e desvio padrão para as variáveis numéricas. Resultados: Dos 192 prontuários, 169 foram considerados para análise. Noventa e dois por cento da amostra foi caracterizada com diagnóstico de DM2 (57±11 anos) e o sexo feminino foi o mais prevalente (66%). Setenta e oito por cento da amostra que continha informações sobre atividade física em seus prontuários era sedentária. Em 83% dos prontuários, não havia recomendação de exercício físico documentado, e os 17% que apresentavam a descrição eram prontuários de pacientes com DM2.  As indicações de “atividade física”, “mudança do estilo de vida” e “hidroginástica” foram os termos mais documentados para recomendação.
