COVID-19 in the municipalities of a macro-region of Minas Gerais and associated factors

Resumo: Introduction: The COVID-19 epidemic in Brazil began in large urban centers and spread to other municipalities over the following months. Objective: To describe the spatial distribution of the incidence of notified cases and mortality by COVID-19 in the municipalities of the Southeastern Minas Gerais health macro-region, according to the Brazilian Institute of Geography and Statistics (IBGE) municipality typology. Material and Methods: Ecological study that had the municipalities of the Southeastern Minas Gerais health macro-region as units of analysis. COVID-19 data were obtained from the virtual platform of the Federal University of Viçosa and the incidence and mortality rates were calculated for each of the municipalities in the macro-region between March 2020 and February 2021. Socioeconomic variables (from the 2010 Census) and health-care variables (from the DATASUS website) were also obtained for these municipalities. Comparisons were made between epidemiological, socioeconomic, and healthcare variables, by types of municipalities, using the non-parametric Kruskal-Wallis test. Results: The distribution of socioeconomic and epidemiological indicators among the municipalities that make up the macro-region was heterogeneous. Predominantly urban municipalities had higher incidence and mortality from COVID-19 than adjacent rural ones. However, adjacent rural municipalities presented, in general, worse socioeconomic indicators when compared to predominantly urban ones. There was no significant difference between adjacent intermediates and other types regarding the indicators evaluated. Conclusion: Although the COVID-19 epidemic first reached the largest, most urbanized and with greater communication with large urban centers municipalities, over time it has been internalized and eventually reached all municipalities in the region.

Durante las siguientes semanas, la enfermedad se extendió por todo el país, afectando a casi todos los municipios, incluidos los más pequeños y remotos.Objetivo: Investigar la distribución espacial de la incidencia y mortalidad por COVID-19 en los municipios de la macrorregión de salud del Sureste de Minas Gerais y la propagación de la epidemia entre los municipios de la región.Material y métodos: Estudio ecológico que tuvo como unidades de análisis a los municipios de la macrorregión de salud Sureste de Minas Gerais. Se obtuvieron las frecuencias semanales de casos y defunciones y se calcularon las tasas de incidencia y mortalidad para los municipios entre marzo de 2020 y febrero de 2021. Variables socioeconómicas (del Censo de 2010) y atención (del Censo de 2010) también se obtuvieron de la web de DATASUS) referentes a estos municipios. Se realizaron comparaciones entre variables epidemiológicas, socioeconómicas y asistenciales, por tipo de municipio (urbano, intermedio y rural).Resultados: Se observó un patrón de dos ondas en el número de casos y muertes relacionadas con la epidemia en la macrorregión Sudeste, una entre marzo y octubre de 2020 y la otra entre noviembre de 2020 y febrero de 2021. La distribución de los indicadores socioeconómicos y epidemiológicos entre los municipios que integran la macrorregión, fue heterogéneo. Los municipios urbanos tuvieron mayor incidencia y mortalidad por COVID-19 que los rurales. Sin embargo, los municipios rurales presentaron, en general, peores indicadores socioeconómicos en comparación con los urbanos.Conclusión: Si bien la epidemia de COVID-19 llegó primero a los municipios más grandes y urbanizados con mayor comunicación con los grandes centros urbanos, con el tiempo se ha internalizado y finalmente llegó a todos los municipios de la región.Introdução: No Brasil, a epidemia de COVID-19 teve início nos grandes centros urbanos e se disseminou para os demais municípios ao longo dos meses seguintes. Objetivo: Descrever a distribuição espacial da incidência de casos notificados e da mortalidade por COVID-19 nos municípios da macrorregião de saúde Sudeste de Minas Gerais segundo a tipologia de municípios do Instituto Brasileiro de Geografia e Estatística (IBGE). Material e Métodos: Estudo ecológico que teve os municípios da macrorregião de saúde Sudeste de Minas Gerais como unidades de análise. Foram obtidos os dados de COVID-19 de plataforma virtual da Universidade Federal de Viçosa e calculadas as taxas de incidência e de mortalidade para cada um dos municípios da macrorregião entre março de 2020 e fevereiro de 2021. Também foram obtidas variáveis socioeconômicas (provenientes do Censo 2010) e assistenciais (provenientes do site do DATASUS) referentes a esses municípios. Foram feitas comparações entre as variáveis epidemiológicas, socioeconômicas e assistenciais, por tipos de município, por meio do teste não-paramétrico de Kruskal-Wallis. Resultados: A distribuição dos indicadores socioeconômicos e epidemiológicos entre os municípios que compõem a macrorregião mostrou-se heterogênea. Os municípios predominantemente urbanos apresentaram maior incidência e mortalidade por COVID-19 do que os rurais adjacentes. Contudo, os municípios rurais adjacentes apresentaram, em geral, piores indicadores socioeconômicos quando comparados aos predominantemente urbanos. Não houve diferença significativa entre os intermediários adjacentes e os demais tipos no que tange aos indicadores avaliados.
