Interrelation between metabolic syndrome and chronic kidney disease

Resumo: Visceral obesity is commonly associated with metabolic syndrome (MS), diabetes mellitus type 2 (DM2) and arterial hypertension, two main causes of chronic kidney disease in the world. However, the role of MS in renal injury in non-diabetic patients has not been fully elucidated. Renal hemodynamic changes among obese individuals are precocious and marked by glomerular hiperfiltration, increased renal plasma flow and increased tubular sodium reabsorption. In addition, lipotoxicity, insulin resistance and inflammation appear to be involved in the kidney damage of obese subjects with MS. The understanding of the mechanisms involved in the MS-related kidney injury might contribute to implement strategies to prevent end-stage kidney disease, in this group of subjects.

Todavia, a participação da SM no desenvolvimento de lesão renal em pacientes não diabéticos tem sido pouco estudada. Alterações funcionais renais em obesos são precoces e caracterizadas por aumento do fluxo plasmático renal, hiperfiltração glomerular e aumento da reabsorção tubular de sódio. Além disso, a resistência insulínica, a inflamação e a lipotoxicidade parecem estar envolvidas na lesão renal associada à obesidade e à SM.
