Being cared for by the homecare service: a rich experience for the aids patient and his caregiver

Resumo: This is an ethnographic study which has the objective of understanding the experience of those who give care and are cared for in the Therapeutic Homecare Service (THS) of the Eduardo de Menezes Hospital. Seventeen informers of both genders aged 25-65 living in the city of Belo Horizonte, State of Minas Gerais, Brazil, informed the study. All the patients had AIDS and were cared for at home by the reference family caregiver. The data was collected through observation of the participants according to Leininger, with ethnographic interviews according to Spradley. The analysis of data obtained was based on Leininger's proposition for ethnographic research. From the analysis of the data, ten cultural descriptors emerged, that made it possible to identify the sub-themes: becoming sick with AIDS - a process that transforms life and relationships; THS provides humanized treatment with professional support, and hospitalization: a difficult and complex experience. These sub-themes constituted the support for the central theme: to be cared by THS: a rich experience for those who give care and those that are cared for. The reflections on the interviews, the sub-themes and the central theme were essential to deepen knowledge about beliefs, values, feelings and needs of those who give care and those who are cared for in the home environment and participated in this study. The anguish felt by the patients and their caregivers in the process of getting AIDS and the frequent hospitalizations led them to perceive THS as a source of support. The improved quality of life is attributed by the informers as a consequence of the care given by the family and by the THS team. All the informers considered themselves privileged in being able to be served by THS.

All the informers considered themselves privileged in being able to be served by THS.Se trata de un estudio etnográfico con la finalidad de entender la experiencia del que vive la internación domiciliaria desde el punto de vista de quién cuida y de quién cuidan en el servicio de atención domiciliaria terapéutica - ADT del Hospital Eduardo de Menezes. Se llevó a cabo con diecisiete informantes, de ambos sexos, con edad entre 25 y 65 años, residentes en Belo Horizonte. Todos los infectados de SIDA eran cuidados en sus respectivas casas por el familiar cuidador asignado para ello. La recogida de datos se basó en la asociación de la observación participativa tal como lo indica Leininger, con entrevista etnográfica según Spradley. El análisis de datos se basó en la propuesta de Leininger para investigación etnográfica. Al analizar los hechos se revelan diez informantes culturales que permiten identificar los siguientes temas: enfermarse de SIDA: un proceso cambiante de la vida y de las relaciones; la ADT proporciona un tratamiento humanizado con respaldo profesional y hospitalización: una experiencia difícil y compleja. Tales temas en derivación constituyen un soporte para el tema central: atención de la ADT: una buena experiencia para quién cuida y para quién cuidan. Las reflexiones sobre las entrevistas, los temas en derivación y el tema central fueron esenciales para profundizar el conocimiento sobre creencias, valores, sentimientos y necesidades de quién cuida y de quién cuidan en el ambiente domiciliario. Las angustias que viven los enfermos y sus cuidadores en el proceso de infección de SIDA y sus internaciones frecuentes los llevaron a sentir que la ADT es una fuente de apoyo. Los informantes sostienen que la mejor calidad de vida se debe a los cuidados brindados por la familia y por el equipo de ADT.Trata-se de um estudo etnográfico cujo objetivo foi compreender a experiência de quem vivencia a internação domiciliar terapêutica na perspectiva de quem cuida e de quem é cuidado no Serviço de Assistência Domiciliar Terapêutica (ADT) do Hospital Eduardo de Menezes. Foi desenvolvido com 17 informantes, de ambos os sexos, com idade entre 25 e 65 anos, residentes em Belo Horizonte. Todos os doentes que tinham aids eram cuidados em suas casas pelo familiar cuidador de referência. A coleta de dados consistiu na associação da observação participante, como preconizada por Leininger, com a entrevista etnográfica, conforme Spradley. A análise dos dados obtidos fundamentou-se na proposição de Leininger para a pesquisa etnográfica. Da análise dos dados emergiram dez descritores culturais que permitiram a identificação dos subtemas: "Adoecer de aids: um processo transformador da vida e dos relacionamentos"; "O ADT proporciona tratamento humanizado e com suporte profissional"; e "A internação hospitalar: uma experiência difícil e complexa". Esses subtemas constituíram o suporte para o tema central: Ser assistido pelo ADT: uma rica experiência para quem cuida e para quem é cuidado. As reflexões sobre as entrevistas, os subtemas e o tema central foram essenciais para aprofundar o conhecimento sobre crenças, valores, sentimentos e necessidades de quem cuida e de quem é cuidado no ambiente domiciliar, participantes deste estudo. As angústias por que passaram os doentes e seus cuidadores no processo de adoecimento da aids e das freqüentes internações os levaram a perceber o ADT como fonte de apoio.
