Percepção do método pilates como estratégia de intervenção terapêutica não farmacológica para idosos com câncer na atenção hospitalar: um olhar sobre a doença experiência

Resumo: A person going through the process of aging must keep being active, through perfecting and seizing health opportunities, participation and security. Since the physiological, morphological and functional changes that occur during the natural aging process can negatively interfere in the functional capacity of the elderly, and the most important changes related to functional independence are changes in body fat, muscle strength, aerobic capacity and flexibility. Furthermore, damage to the psychological and social dimensions of individuals can further impair autonomy and independence. If, with advancing age, individuals tend to become more sedentary, the situation worsens with the diagnosis of a serious illness. Acknowledging the importance of physical exercise as a non-medicated and non-pharmacological therapeutic intervention strategy for elders in Health Care, new exercise proposals and methodologies are suggested for this intervention. In this context, the Pilates Method is brought forth which, due to its characteristics based on Contrology and global work in all exercises, also for looking at the person beyond quantitative data, focusing on a body-mind-spirit interaction, it has been indicated by several professionals, gaining adepts all over the world. However, the method is still little studied when compared to other exercise methods, mainly if we consider health care and psychosocial aspects. This study has a mainly qualitative approach and its goal is to observe and understand the subjects’ experience. The research universe was composed by 12 individuals with a cancer diagnosis who were inserted in Health Care. These individuals have volunteered to participate in the Pilates Extension Project offered by the responsible for this research in a Belo Horizonte hospital. As part of the understanding of how physical exercise, in this case the Mat Pilates, can contribute to life quality in the social and psychological context; and how these subjects interpret their experience, a semi structured interview and a participant observation were made. The interviews were made before the beginning of the Pilates Method intervention and three months after its conclusion. The technique used for the interview analysis was the dialectical hermeneutics, which makes the synthesis of the comprehensive and critical processes searching for the meaning comprehension that occurs in the communication between human beings, having language as its central core. The identified analysis categories were: “Pilates: an opportunity”; “Interpreting health”; “Interpreting the disease”; “Disease: news x acceptance”; “Pilates is movement”; “Pilates and socialization” and “Pilates: body-mind-spirit”. It is concluded that having a positive experience in hospital care and establishing new bonds and new concepts of belonging when experiencing the Pilates Method as a non-pharmacological therapeutic strategy was beneficial for the general quality of life of individuals. The act of getting involved and belonging to groups with which they have an affinity gave a new meaning to their days and generated a new strength in such difficult times, one day someone was inspired and the next became a reason for inspiration. And in all this scenario the Pilates Method has become a lifestyle transforming the body, mind and spirit of practitioners.

Nesta fase acontecem mudanças fisiológicas, morfológicas e funcionais que interferem negativamente na capacidade funcional do idoso, e as que causam maior impacto na independência funcional são a adiposidade corporal, a força muscular, a capacidade aeróbica e a flexibilidade. Estas mudanças na dimensão biológica acarretam prejuízos nas dimensões psicológicas e sociais, que juntas podem prejudicar ainda mais a autonomia e independência do idoso. Este quadro pode se agravar ainda mais com o diagnóstico de uma doença grave. Este contexto mostra a importância do idoso se manter ativo, através de um processo de aperfeiçoamento e aproveitamento das oportunidades para a saúde, participação e segurança, com o objetivo de não perder a qualidade de vida ao longo da velhice. O exercício físico pode ser uma importante estratégia de intervenção terapêutica não farmacológica ou não medicamentosa para idosos, e pode ser mais importante ainda na Atenção Hospitalar, o que leva a busca de novas propostas de exercícios e métodos para essa intervenção. O Método Pilates aparece como uma ótima opção, devido às suas características baseadas na contrologia e no trabalho global em todos os exercícios, e no olhar a pessoa para além de dados quantitativos, com foco na interação corpo-mente-espírito. O Método Pilates tem ganhado diversos adeptos pelo mundo, e tem sido indicado pelos profissionais da saúde para o tratamento e prevenção de idosos. Entretanto, poucos estudos têm investigado a eficácia deste método de forma mais controlada, principalmente relativo à população idosa na Atenção Hospitalar e que também analise aspectos psicológicos e sociais. Este estudo tem como objetivo compreender os efeitos do Método Pilates na experiência de idosos na Atenção Hospitalar, quando utilizado como uma estratégia terapêutica não farmacológica. O universo de pesquisa foi composto por 12 indivíduos, com diagnóstico de câncer e inseridos na Atenção Hospitalar. Estes sujeitos se voluntariaram a participar do Projeto de extensão em Pilates oferecido através dos responsáveis por essa pesquisa no Hospital do Instituto de Previdência dos Servidores do Estado de Minas Gerais (IPSEMG). Como parte do entendimento de como o Método Pilates, pode contribuir para qualidade de vida no contexto social e psicológico; e como esses sujeitos interpretam essas experiências, foi realizada uma entrevista semiestruturada e observação participante. As entrevistas foram realizadas antes do início da intervenção com o Método Pilates e três meses após a intervenção. A técnica utilizada para análise das entrevistas foi a hermenêutica dialética que faz a síntese dos processos compreensivos e críticos buscando a compreensão de sentido que se dá na comunicação entre seres humanos, tendo na linguagem seu núcleo central. Os resultados revelaram as categorias de análise identificadas à posteriori: “Pilates: uma oportunidade”; “Interpretando a saúde”; “Interpretando a doença”; “Doença: notícia x aceitação”; “Pilates é movimento”; “Pilates e socialização” e “Pilates: corpo-mente-espírito”. Conclui-se que o fato de vivenciar experiências positivas na atenção hospitalar e estabelecer novos laços e novos conceitos de pertencimento ao vivenciar o Método Pilates como estratégia terapêutica não farmacológica foi benéfico para a qualidade de vida geral dos indivíduos. O ato de se envolver e pertencer a grupos com os quais possuem afinidade deu um novo sentido aos seus dias e gerou uma nova força em tempos tão difíceis, um dia se inspirava alguém e no outro se tornava um motivo de inspiração.
