Construção e validação de protocolo da CIPE® para a prevenção e tratamento da pessoa com lesão por pressão

Resumo: | Introduction: Pressure injuries are considered a public health problem, due to their high incidence. Objective: To validate a CIPE® Protocol built for the prevention and treatment of people with pressure injuries in the light of the theoretical model of Horta. Method: It was a qualitative and quantitative methodological research, carried out in the following stages: 1) integrative review and mapping of terms with ICNP® 2017, 2) content validation of nursing interventions with specialist nurses, 3) preparation of the flowchart for using the protocol, and 4) structuring of the ICNP® protocol for the prevention and treatment of pressure injuries. From the first stage, 31 articles were extracted, and 74 nursing interventions were identified. These interventions were mapped with CIPE 2017 and classified as equal, similar, more comprehensive, more restricted and there is no agreement. The instrument was structured and sent to perform content validation by 30 specialist nurses, in order to assess whether or not they were applicable to the prevention and treatment of people with pressure injuries. Data collection took place from November 2018 to May 2019. Data analysis was performed using the statistical package R. The interventions considered applicable were those that reached the Content Validity Index greater than or equal to 0.80 in the validation, p-value less than or equal to 0.05 and Cronbach's alpha above 0.80. This study, before being started, was assessed by CEP / HULW / UFPB and approved by CAAE 96366318.8.0000.5183, under opinion no. 2,965,744. Results: It was evidenced with the statistical analysis that 93.67% of the statements of nursing interventions had a Content Validation Index greater than 80%, the p-value for the proportion equality test was less than 0.05 and high levels of Cronbach's alpha. Five 6.33% interventions had a Content Validity Index below 80%, therefore, they were removed from the instrument. A care flow chart was elaborated and the protocol was structured with 74 nursing interventions considered applicable in the prevention and treatment of people with pressure injuries. Conclusion: The findings of this study will strengthen clinical practice, teaching and the scientific community in the prevention and treatment of people with pressure injuries. |

Objetivo: Validar un protocolo CIPE® creado para la prevención y el tratamiento de personas con lesiones por presión a la luz del modelo teórico de Horta. Método: Fue una investigación metodológica cualitativa y cuantitativa, realizada en las siguientes etapas: 1) revisión integradora y mapeo de términos con ICNP® 2017, 2) validación de contenido de intervenciones de enfermería con enfermeras especializadas, 3) preparación del diagrama de flujo para usar el protocolo, y 4) estructuración del protocolo ICNP® para la prevención y el tratamiento de lesiones por presión. Desde la primera etapa, se extrajeron 31 artículos y se identificaron 74 intervenciones de enfermería. Estas intervenciones se mapearon con CIPE 2017 y se clasificaron como iguales, similares, más completas, más restringidas y no hay acuerdo. El instrumento fue estructurado y enviado para realizar la validación del contenido por 30 enfermeras especializadas, con el fin de evaluar si eran aplicables o no a la prevención y el tratamiento de personas con lesiones por presión. La recopilación de datos se realizó entre noviembre de 2018 y mayo de 2019. El análisis de datos se realizó utilizando el paquete estadístico R. Las intervenciones consideradas aplicables fueron aquellas que alcanzaron un Índice de validez de contenido mayor o igual a 0,80 en la validación. valor p menor o igual a 0.05 y alfa de Cronbach por encima de 0.80. Este estudio, antes de comenzar, fue evaluado por CEP / HULW / UFPB y aprobado por CAAE 96366318.8.0000.5183, según la opinión no. 2.965.744. Resultados: Del análisis estadístico resultó evidente que el 93,67% de las declaraciones de intervenciones de enfermería tenían un índice de validación de contenido superior al 80%, el valor p para la prueba de igualdad de proporciones era inferior a 0,05 y altos niveles de alfa de Cronbach. Cinco intervenciones del 6,33% tenían un índice de validez de contenido inferior al 80%, por lo tanto, se eliminaron del instrumento. Se elaboró un diagrama de flujo de atención y el protocolo se estructuró con 74 intervenciones de enfermería consideradas aplicables en la prevención y el tratamiento de personas con lesiones por presión. Método: Tratou-se de uma pesquisa metodológica de caráter quali-quantitativa, realizada em quadro etapas: 1) revisão integrativa e mapeamento dos termos com a CIPE® 2017, 2) validação de conteúdo das intervenções de enfermagem com os enfermeiros especialistas, 3) elaboração do fluxograma de utilização do protocolo, e 4) estruturação do protocolo da CIPE® para prevenção e tratamento das lesões por pressão. Da primeira etapa, foram extraídos 31 artigos, e identificadas 74 intervenções de enfermagem. Essas intervenções formaram mapeadas com a CIPE 2017 e classificadas como igual, similar, mais abrangente, mais restrita e não existe concordância. O instrumento foi estruturado e enviado para realizar a validação de conteúdo por 30 enfermeiros especialistas, com intuito de avaliar se eram aplicáveis ou não à prevenção e tratamento de pessoas com lesão por pressão. A coleta de dados ocorreu de novembro de 2018 a maio de 2019. A análise dos dados foi realizada utilizando o pacote estatístico R. As intervenções consideradas aplicáveis foram aquelas que atingiram na validação o Índice de Validade de Conteúdo maior ou igual a 0,80, p-valor menor ou igual a 0,05 e Alfa de Cronbach acima de 0,80. Este estudo, antes de ser iniciado, foi apreciado pelo CEP/HULW/UFPB e aprovado pelo CAAE 96366318.8.0000.5183, sob parecer no. 2.965.744. Resultados: Evidenciou-se com a análise estatística que 93,67% dos enunciados de intervenções de enfermagem tiveram o Índice de Validação de Conteúdo superior a 80%, o p-valor para o teste da igualdade da proporção foi inferior a 0,05 e altos níveis do alfa de Cronbach. Cinco intervenções 6,33% tiveram o Índice de Validade de Conteúdo inferior a 80%, portanto, foram retiradas do instrumento. Foi elaborado um fluxograma de atendimento e estruturado o protocolo com 74 intervenções de enfermagem consideradas aplicáveis na prevenção e tratamento das pessoas com lesão por pressão.
