Efeito crônico do método pilates nas dimensões morfológicas, neurológicas e metabólicas de pessoas obesas recuperadas da Covid-19: ensaio clínico controlado

Resumo: Introduction: Covid-19 and its etiological agent, SARS-CoV-2 identified in December 2019 in China, in the city of Wuhan, has spread throughout the world, despite the strategies adopted by the Chinese government to prevent this epidemiological phenomenon . Covid-19 is an epidemic disease with a high associated mortality rate, with, among other consequences, reduced breathing capacity and shortness of breath. Physical exercise proved to be a strong ally as an auxiliary therapy for Covid-19 and among the training methods, the Pilates. Objective: To evaluate and compare the effects of the Mat Pilates method on lung function, respiratory muscle strength, motor skills, mass and Body Mass Index (BMI), biochemical variables, quality of life (QoL) and pain in obese people recovered from covid -19 and obese people who have not contracted covid-19. Methodology: 54 subjects (18 to 60 years old), obese (BMI: >30), without locomotion disorders and with controlled pre-existing diseases (diabetes and hypertension) participated in the study, divided into 2 experimental groups: Solo Pilates Group with Covid -19 (GPS+Covid-19; n = 26) and, Solo Pilates Group without Covid-19 (GPS; n = 27) who performed, 2x/week, 36 sessions of the Pilates method, whose training was divided into two phases: A (22 exercises) and B (18 exercises). Data were analyzed in SPSS (23.0), using the following Student's t test and Wilcoxon for intragroup comparison, and independent Student's t test and Mann-Whitney test, in addition to Fisher's test for comparison between pairs, considering P≤0,05. Results: Significant differences were observed in the variables Forced Vital Capacity (FVC: P=0.001), Forced Expiratory Volume (FEV1: P=0.001) and in the FEV1/VC% ratio: P<0.001) in the comparison before and after 36 sessions of GPS+Covid-19 exercise, and also in the comparison of the FVC: (P=0.023), the FEV1 (P=0.040) and the FEV1/VC% ratio (P=0.035) in the GPS. Regarding the biochemical variables, significant increases were found for red blood cells (P<0.001), platelets (P<0.001), albumin (P=0.011), HDL cholesterol (P=0.030) and vitamin D (P<0.001), for the variables uric acid (P<0.003), TGO (P=0.007) and TGP (P=0.028) a significant decrease, in motor tests a significant improvement was found in flexibility (P<0.001), balance (P=0.025), agility (P<0.001), cardiorespiratory capacity (P=0.002) and decrease in pain (P<0.001), with no significant decrease in body mass and BMI for GPS+Covid-19. For GPS, red blood cells (P<0.001), HDL cholesterol (P<0.001) and albumin (P=0.032) had significant increases, while for: uric acid (P<0.001), triglycerides (P<0.001) and T3 total (P<0.001) there was a significant decrease. For 7 quality of life, in the GPS+covid-19, significant differences were found in the physical (P=0.018), psychological (P<0.001), social relationships (P<0.001), environment (P<0.001) and self-assessment domains for QOL (P<0.001), while for GPS there was only a significant difference in the environment domain (P<0.001). In the intergroup comparison, no significant differences were found in FVC values (P=0.721). FEV1 (P=0.323) and FEV1/CV% (P=0.239). Conclusion: The results showed that the Pilates method had an effect on the recovery of lung function, respiratory muscle strength and biochemical components, red blood cells, platelets, uric acid, albumin, HDL cholesterol, total T3, free T4, TGO, TPG and vitamin D In assessing QoL, the Pilates method improved in the domains: physical, psychological, social relationships, environment and self-assessment.

Introducción: El Covid-19 y su agente etiológico, el SARS-CoV-2 identificado en diciembre de 2019 en China, en la ciudad de Wuhan, se ha extendido por todo el mundo, a pesar de las estrategias adoptadas por el gobierno chino para prevenir este fenómeno epidemiológico. El Covid-19 es una enfermedad epidémica con una alta tasa de mortalidad asociada, con, entre otras consecuencias, reducción de la capacidad respiratoria y dificultad para respirar. El ejercicio físico demostró ser un fuerte aliado como terapia auxiliar para el Covid-19 y entre los métodos de entrenamiento, el método Pilates. Objetivo: Evaluar y comparar los efectos del método Mat Pilates sobre la función pulmonar, fuerza muscular respiratoria, motricidad, masa e Índice de Masa Corporal (IMC), variables bioquímicas, calidad de vida (CV) y dolor en personas obesas recuperadas de covid -19 y personas obesas que no hayan contraído covid-19. Metodología: Participaron del estudio 54 sujetos (18 a 60 años), obesos (IMC: >30), sin trastornos de la locomoción y con enfermedades preexistentes controladas (diabetes e hipertensión), divididos en 2 grupos experimentales: Grupo Solo Pilates con Covid -19 (GPS+Covid-19; n = 26) y, Grupo Solo Pilates sin Covid-19 (GPS; n = 27) que realizaron, 2x/semana, 36 sesiones del método Pilates, cuyo entrenamiento se dividió en dos fases: A (22 ejercicios) y B (18 ejercicios). Los datos fueron analizados en SPSS (23.0), utilizándose las siguientes pruebas t de Student y Wilcoxon para comparación intragrupo, y t de Student independiente y prueba de Mann-Whitney, además de la prueba de Fisher para comparación entre pares, considerando P≤0,05. Resultados: Se observaron diferencias significativas en las variables Capacidad Vital Forzada (FVC: P=0,001), Volumen Espiratorio Forzado (VEF: P=0,001) y en la relación VEF/VC%: P<0,001) en la comparación antes y después de 36 años. sesiones de ejercicio GPS+Covid-19, y también en la comparación de la FVC: (P=0,023), el VEF1 (P=0,040) y el cociente VEF1/VC% (P=0,035) en el GPS. En cuanto a las variables bioquímicas, se encontraron aumentos significativos de glóbulos rojos (P<0,001), plaquetas (P<0,001), albúmina (P=0,011), colesterol HDL (P=0,030) y vitamina D (P<0,001), por las variables ácido úrico (P<0,003), TGO (P=0,007) y TGP (P=0,028) una disminución significativa, en las pruebas motoras se encontró una mejora significativa en flexibilidad (P<0,001), equilibrio (P=0,025), agilidad (P<0,001), capacidad cardiorrespiratoria (P=0,002) y disminución del dolor (P<0,001), sin disminución 9 significativa de masa corporal e IMC para GPS+Covid-19. Para GPS, glóbulos rojos (P<0,001), colesterol HDL (P<0,001) y albúmina (P=0,032) tuvieron incrementos significativos, mientras que para: ácido úrico (P<0,001), triglicéridos (P<0,001) y T3 total (P<0,001) hubo una disminución significativa. Para la calidad de vida, en el GPS+covid-19 se encontraron diferencias significativas en el aspecto físico (P=0,018), psicológico (P<0,001), relaciones sociales (P<0,001), entorno (P<0,001) y autoconocimiento. los dominios de evaluación para CDV (P<0,001), mientras que para GPS solo hubo una diferencia significativa en el dominio ambiental (P<0,001). En la comparación intergrupos no se encontraron diferencias significativas en los valores de FVC (P=0,721). VEF1 (P=0,323) y VEF1/%CV (P=0,239). Conclusión: Los resultados mostraron que el método Pilates tuvo efecto en la recuperación de la función pulmonar, fuerza muscular respiratoria y componentes bioquímicos, glóbulos rojos, plaquetas, ácido úrico, albúmina, colesterol HDL, T3 total, T4 libre, TGO, TPG y vitamina D en la evaluación de la CV, el método Pilates mejoró en los dominios: físico, psicológico, relaciones sociales, ambiente y autoevaluación.NenhumaIntrodução: A Covid-19 é uma doença com caráter epidêmico com elevada taxa de mortalidade associada, tendo dentre outras consequências, a redução da capacidade respiratória e dispnéia. Objetivo: Avaliar e comparar os efeitos do método Pilates Solo sobre a função pulmonar, força muscular respiratória, capacidades motoras, massa e Índice de Massa Corporal (IMC), variáveis bioquímicas, qualidade de vida (QV) e dor em pessoas obesas recuperadas da covid-19 e obesos que não contraíram a covid-19. Para o GPS, as hemácias (P<0,001), colesterol HDL (P<0,001) e albumina (P=0,032) tiveram aumentos significantes, enquanto, para: ácido úrico (P<0,001), triglicerídeos (P<0,001) e T3 total (P<0,001) houve uma diminuição significante. Para a qualidade de vida, no GPS+covid-19, foram encontradas diferenças significantes nos domínios físico 5 (P=0,018), psicológico (P<0,001), relações sociais (P<0,001), meio ambiente (P<0,001) e autoavaliação da QV (P<0,001), enquanto para o GPS só houve diferença significante no domínio meio ambiente (P<0,001). Na comparação intergrupos não foram encontradas diferenças significantes nos valores da CVF (P=0,721). VEF1 (P=0,323) e VEF1/CV% (P=0,239). Conclusão: Os resultados mostraram que o método Pilates teve efeito na recuperação da função pulmonar, da força muscular respiratória e nos componentes bioquímicos, hemácias, plaquetas, ácido úrico, albumina, colesterol HDL, T3 total, T4 livre, AST, ALT e vitamina D.
