Determinantes sociais da violência contra idosos hospitalizados

Resumo: Introduction: Violence against the Elderly is configured as a socially determined problem, which affects this population with high prevalence in all countries, with emphasis on those in development. Thus, it is essential to analyze the relationship between violence and Social Determinants, such as sex, marital status, education level, housing arrangement, work activity, income and health self-assessment, aiming to direct health actions that enable the reduction of social inequities. The determinants will be studied by the Dahlgren and Whitehead model and the ecological model. The models converge because they have a classification in three major axes: macrodeterminants, intermediate determinants and microdeterminants for Dahlgren and Whitehead; macro, meso and microsystem for the ecological. Objective: to analyze the association of social determinants with violence against the elderly. Method: systematic review with metaanalysis, performed on the following databases PubMed, CINAHL, Scopus, PsycINFO, Web of Science and LILACS, using the following descriptors: Associated Factors, Elderly, Aged, Elder Abuse, Violence. The sample comprised 46 articles. Then, a quantitative, multicenter and cross-sectional study was carried out in two university hospitals in Paraíba, with 323 elderly people, from July 2019 to February 2020. The instruments used for data collection were: Brazil Old Age Schedule, Hwalek-Sengstock Elder Abuse Screening Test and Conflict Tactics Scale-1 to collect data that were analyzed using descriptive and inferential statistics. Results: the systematic review pointed out that all the factors evaluated are configured as determinants of violence, with the exception of marital status, which pointed out that having a relationship is a protective factor for violence (OR = 0.86). Still, self-rated health was the determinant with the strongest association (OR = 1.35). In the quantitative study, it was observed that there was a significant association between risk of violence and the female sex (p = 0.004), living with grandchildren (p = 0.025) and having four or more diseases (p <0.001), in addition to income and physical violence (p = 0.048). Belonging to the female sex, being literate and having more diseases increases the risk of violence. Conclusion: the intermediate determinants were more prominent in the systematic review, so that classifying health as poor or regular increases the elderly's exposure to situations of violence. Furthermore, for the research article, the determinants that most strongly influenced situations of violence were, for the most part, those that make up the macrosystem, considering the income and number of diseases that the elderly person has.

Los determinantes serán estudiados por el modelo Dahlgren y Whitehead y el modelo ecológico. Los modelos convergen porque tienen una clasificación en tres ejes principales: macrodeterminantes, determinantes intermedios y microdeterminantes para Dahlgren y Whitehead; macro, meso y microsistema para lo ecológico. Objetivo: analizar la asociación de los determinantes sociales con la violencia contra las personas mayores. Método: revisión sistemática con metanálisis, realizada en las siguientes bases de datos PubMed, CINAHL, Scopus, PsycINFO, Web of Science y LILACS, utilizando los siguientes descriptores: Factores asociados, ancianos, ancianos, abuso de ancianos, violencia. La muestra comprendió 46 artículos. Luego, se realizó un estudio cuantitativo, multicéntrico y transversal en dos hospitales universitarios de Paraíba, con 323 personas mayores, de julio de 2019 a febrero de 2020. Los instrumentos utilizados para la recopilación de datos fueron: Brasil Vejez Schedule, Hwalek-Sengstock Elder Abuse Screening Test y Conflict Tactics Scale-1 para recopilar datos que se analizaron mediante estadísticas descriptivas e inferenciales. Resultados: la revisión sistemática señaló que todos los factores evaluados están configurados como determinantes de la violencia, con la excepción del estado civil, que señaló que tener una relación es un factor protector para la violencia (OR = 0,86). Aún así, la salud autoevaluada fue el determinante con la asociación más fuerte (OR = 1.35). En el estudio cuantitativo, se observó que había una asociación significativa entre el riesgo de violencia y el sexo femenino (p = 0.004), viviendo con nietos (p = 0.025) y teniendo cuatro o más enfermedades (p <0.001), además de los ingresos y violencia física (p = 0.048). Pertenecer al sexo femenino, saber leer y escribir y tener más enfermedades aumenta el riesgo de violencia. Conclusión: los determinantes intermedios fueron más prominentes en la revisión sistemática, por lo que clasificar la salud como pobre o regular aumenta la exposición de los ancianos a situaciones de violencia. Os determinantes serão estudados pelo modelo de Dahlgren e Whitehead e o modelo ecológico. Os modelos confluem por apresentar uma classificação em três grandes eixos: macrodeterminantes, determinantes intermediários e microdeterminantes para o de Dahlgren e Whitehead; macro, meso e microssistema para o ecológico. Objetivo: analisar a associação dos determinantes sociais com a violência contra a pessoa idosa. Método: revisão sistemática com metanálise, realizada nas seguintes bases PubMed, CINAHL, Scopus, PsycINFO, Web of Science e LILACS, utilizando os seguintes descritores: Associated Factors, Elderly, Aged, Elder Abuse, Violence. Compuseram a amostra 46 artigos. Em seguida, foi desenvolvido um estudo quantitativo, multicêntrico e de corte transversal, em dois hospitais universitários da Paraíba, com 323 idosos, no período de julho de 2019 a fevereiro de 2020. Utilizou-se para coleta de dados os instrumentos: Brazil Old Age Schedule, Hwalek-Sengstock Elder Abuse Screening Test e Conflict Tactics Scale-1 para coleta dos dados que foram analisados mediante estatística descritiva e inferencial. Resultados: a revisão sistemática apontou que todos os fatores avaliados se configuram como determinantes da violência, com exceção do estado civil que apontou que ter um relacionamento é um fator de proteção para a violência (OR=0.86). Ainda, a autoavaliação de saúde foi o determinante com maior força de associação (OR=1.35). No estudo quantitativo, observou-se que houve associação significativa entre risco para violência e o sexo feminino (p=0,004), residir com netos (p=0,025) e ter quatro ou mais doenças (p<0,001), além de renda e a violência física (p=0,048). Pertencer ao sexo feminino, ser alfabetizado e ter maior número de doenças aumenta o risco para violência. Conclusão: os determinantes intermediários foram de maior destaque na revisão sistemática, de forma que classificar a saúde como ruim ou regular aumenta a exposição do idoso a situações de violência.
