A síndrome metabólica (SM) é uma desordem relacionada à obesidade de frequência elevada. Dados publicados na literatura médica apontam a obesidade como um fator de risco para cefaleia crônica. A potencial interligação entre essas duas doenças tem sido objeto de recentes estudos por diversas razões, principalmente por que a obesidade pode interferir na história natural da migrânea. Este estudo se propõe identificar a prevalência das cefaleias primárias em pacientes obesos com e sem síndrome metabólica, e, a partir dessa identificação, verificar qual o tipo ou os tipos mais frequentes de cefaleia. Noventa e um pacientes obesos, com e sem síndrome metabólica, foram incluídos no estudo. Para diagnóstico da SM, utilizaram-se critérios estabelecidos pela I Diretriz Brasileira de Diagnóstico e Tratamento da SM (2005) que adotam os critérios definidos pelo NCEP-ATP III (2001). Todos os pacientes passaram por entrevista clínica para fins de obter variáveis demográficas (idade e sexo) e antropométricas (peso, altura, CC e IMC), dados clínicos sobre doenças prévias, dados laboratoriais constantes nos prontuários e as características das cefaleias (tipo, frequência e sintomas associados). As cefaleias primárias foram avaliadas por meio de entrevista estruturada e foram empregados os critérios da Classificação Internacional das Cefaleias (2ª edição, 2004) para diagnóstico e classificação da mesma. Trata-se de um estudo epidemiológico descritivo de corte transversal. As análises estatísticas foram realizadas utilizando-se χ². Entre os participantes 87,9% (80/91) corresponderam ao sexo feminino e 12,1% (11/91) ao sexo masculino. Síndrome metabólica foi diagnosticada em 63,7% (58/91) dos pacientes. Todos os participantes (100%) receberam algum diagnóstico de cefaleia primária. A cefaleia do tipo tensional (CTT) foi a mais frequente em ambos os grupos em uma frequência de 60,3% (35/91) nos pacientes com SM vs 54,51% (22/91) nos pacientes sem SM. Não houve diferença significativa dos tipos de cefaleias (CTT e MSA) entre os pacientes com (58/91) e sem SM (33/91) (p= 0,59). A migrânea com aura ocorreu apenas no grupo com SM em uma frequência de 1,1%. Verificou-se diferença significante entre os tipos de cefaleias (CTT e MSA) e o sexo (p= 0,023). Não houve diferença significativa entre os tipos de cefaleias (CTT e MAS) em relação aos critérios que definem a SM no grupo com essa síndrome [elevação da PA (p= 0,2587); glicemia elevada (>110 mg/dL) (p= 0,855) e triglicerídeo elevado (p= 0,689) e níveis de HDL – colesterol (p= 1)], porém houve diferença dos níveis reduzidos de HDL – colesterol (p=1) entre mulheres com cefaleia do tipo migranosa e não migranosa. O diagnóstico de cefaleia é comum em pacientes obesos. Cefaleias primárias apresentam importante relação com a obesidade e com a SM, porém a presença dessa, bem como a presença dos seus componentes isoladamente, não interfere na prevalecia do tipo de cefaleia. O único parâmetro da SM que pode estar relacionado com a frequência de migrânea é o nível baixo HDL – colesterol.Headache and obesity are frequent diseases in the general population and are influenced by genetic and environmental factors, causing an impact on quality of life and significant costs to society. Metabolic syndrome (MS) is a disorder related to high frequency obesity. Data published in the medical literature point to obesity as a risk factor for chronic headache. The potential interconnection between these two diseases has been the object of recent studies for several reasons, mainly because obesity can interfere in the natural history of migraine. This study aims to identify the prevalence of primary headaches in obese patients with and without metabolic syndrome, and from this identification, to verify the type or the most frequent types of headache. Ninety-one obese patients, with and without metabolic syndrome, were included in the study. For the diagnosis of MS, criteria established by the I Brazilian Guideline on Diagnosis and Treatment of MS (2005) were adopted, adopting the criteria defined by NCEP-ATP III (2001). All patients underwent a clinical interview in order to obtain demographic variables (age and gender) and anthropometric variables (weight, height, WC and BMI), clinical data on previous diseases, laboratory data in the charts and characteristics of headaches and associated symptoms). Primary headaches were evaluated through a structured interview and the criteria of the International Classification of Headaches (2nd edition, 2004) were used for diagnosis and classification. This is a cross-sectional descriptive epidemiological study. Statistical analyzes were performed using χ². Among the participants, 87.9% (80/91) were female and 12.1% (11/91) were male. Metabolic syndrome was diagnosed in 63.7% (58/91) of the patients. All participants (100%) received some diagnosis of primary headache. Tension-type headache (TTH) was the most frequent in both groups at a frequency of 60.3% (35/91) in patients with MS vs 54.51% (22/91) in patients without MS. There was no significant difference in types of headache (TTH and MWA) among patients with (58/91) and no MS (33/91) (p= 0.59). Migraine with aura occurred only in the MS group at a frequency of 1.1%. There was a significant difference between types of headache (TTH and MWA) and gender (p = 0.023). There was no significant difference between the types of headache (TTH and MWA) in relation to the criteria that define MS in the group with this syndrome [BP elevation (p= 0.2587); (p= 0.865) and high triglyceride levels (p = 0.689) and HDL - cholesterol levels (p= 1)), but there was a difference in the reduced levels of HDL - cholesterol (p= 1) among women with migraine and non-migraine headache. The diagnosis of headache is common in obese patients. Primary headaches have an important relationship with obesity and MS, but the presence of this disease, as well as the presence of its components alone, does not interfere with the prevalence of headache type.
