CamposTese (doutorado) - Universidade Federal do Paraná, Setor de Ciencias da SaúdeResumo: A presente tese tem como objetivo descrever os achados oftalmológicos, dermatológicos, de microscopia óptica (MO) e de microscopia eletrônica de varredura (MEV) de uma nova distrofia macular associada à síndrome dos cabelos anágenos frouxos (SCAF). Uma família de onze pacientes, quatro deles afetados ocularmente, foram examinados. Sete pacientes foram submetidos ao exame oftalmológico completo, teste de cores (Ishihara e Farnsworth D-15), ecografia, angiografia, campo visual (CV), topografia de papila (TOPSS), eletro-oculograma (EOG), eletrorretinograma (ERG), exame laboratorial e dermatológico, teste de suor, MO e MEV dos fios de cabelo. Em quatro pessoas, filhos dos pacientes afetados, realizamos somente o exame oftalmológico, a MO e a MEV dos fios de cabelo. Dos quatro pacientes afetados ocularmente, em duas irmãs encontramos no fundo de olho dispersões pigmentares em pólo posterior da retina, com displasia macular (coloboma). Em dois de seus irmãos foram encontradas as mesmas alterações oculares, com maior pigmentação e coloração amarelada em área macular e sem coloboma. Os resultados do EOG e ERG estava dentro dos limites de normalidade em três pacientes sem alterações oculares; por outro lado, o EOG foi subnormal e o ERG foi subnormal com tempo de culminação diminuído (rápido) na fase escotópica dos pacientes com a distrofia macular, sugerindo que a alteração dessa distrofia se encontra em nível do epitélio pigmentado da retina (EPR). Devido a esse comportamento peculiar do tempo de culminação do ERG, e por não estar descrito na literatura até o presente momento, este achado pode ser considerado como fundamental para o diagnóstico desta nova distrofia. Portanto, a denominação adequada sugerida é de Distrofia Retiniana com Onda Rápida Escotópica (DRORE). A avaliação na MO e na MEV dos fios de cabelo dos indivíduos afetados ocularmente confirmaram ser SCAF. Os pacientes heterozigotos, mãe e o irmão mais velho dos pacientes afetados ocularmente, apresentaram exame oftalmológico normal, exceto que na MO e na MEV dos fios de cabelo, encontramos algumas semelhanças com os indivíduos afetados e características da SCAF. A análise genealógica da família demonstrou tratar-se de herança autossômica recessiva com expressão parcial no heterozigoto. Há somente uma relato na literatura internacional da associação de SCAF e coloboma ocular e nenhuma na literatura latino-americana (LILACs) sobre a SCAF. Neste trabalho descrevemos os achados da DRORE associada à SCAF, distrofia cujos achados fundoscópicos são diferentes entre homens e mulheres e que têm ERG característico. Por se tratar do primeiro relato na literatura, os achados descritos sugerem fortemente que essa associação pode ser uma nova entidade nosológica dentro do grupo B das displasias ectodérmicas.Abstract: The present work aims to describe the ophthalmological, dermatological, light microscopy (LM) and scanning electron microscopy (SEM) findings of a new macular dystrophy associated to loose anagen hair syndrome (LAHS). Eleven patientes of the same family, four with ocular abnormalities, were examined. Seven patients had been submitted to a complete ophthalmological examination, color test (Ishihara and Farnsworth D-15), ultrasonography, angiography, visual field (VF), optic disc topography (TOPSS), electro oculogram (EOG), electroretinogram (ERG), laboratory and dermatological exams, sweat testing, LM and SEM of the scalp hair. In four people, children of affected patients, examination was restricted to a ophthalmological examination and LM and the MEV of the scalp hair. Of the four affected patients with ocular findings, two affected sisters demonstrated pigmentary dispersions in the posterior pole of retina with macular dysplasia (coloboma). Two brothers showed pigmentary dispersions in the posterior pole, with greater pigmentation and a yellow aspect of the macular area without coloboma. The results of the EOG and ERG were within normal limits in three patients without ocular abnormalities. On the other hand, EOG was subnormal and ERG was subnormal with short implicit time (fast) in the scotopic phase of the patients with the macular dystrophy, suggesting that the site of this dystrophy is at the level of the retinal pigment epithelium (RPE). Due to to this peculiar behavior of the implicit time of the wave in the ERG, without any previous description in the literature, such finding can be considered as fundamental to make the diagnosis of this new dystrophy. Thus, an adequate denomination for such entity could be Scotopic Fast Wave Retinal Dystrophy (SFWRD). The analysis of MO and SEM of scalp hair in the eye affected individuals confirmed the LAHS. In heterozygous patients, the mother and the oldest brother of the eye affected patients presented with normal ophthalmological examination, but in LM and SEM of scalp hair, some similarities with the affected individuals were found to be characteristic of the LHAS. The pedigree analysis of the family suggested an autosomal recessive form of inheritance with partial expression in the heterozygote. There is only one report in the international literature of the LAHS associated to ocular coloboma and nothing in Latin American literature (LILACs) about LAHS. In this work we describe the findings of the SFWRD associated to the LAHS, dystrophy which fundoscopy findings differ from men to women and with characteristic ERG.
