VULNERABLE TERRITORIES TO THE OCCURRENCE OF TUBERCULOSIS/HIV COINFECTION IN A TUBERCULOSIS ENDEMIC MUNICIPALITY

Resumo: | Objective: To analyze the territories vulnerable to the occurrence of TB/HIV co-infection in an endemic scenario for tuberculosis in the Brazilian Northeast. Methods: Ecological, descriptive and exploratory study, carried out in Imperatriz (MA), Northeast of Brazil. The municipality's census sectors were considered as ecological units of analysis. Cases of TB/HIV co-infection were included, based on tuberculosis notification forms registered with the Disease Notification System (SINAN) between 2010 and 2019. Spatial area analysis and Kernel analysis were carried out. The cases were geocoded using TerraView software, version 4.2.2, and tools such as Batch Geocode and GPS. Results: During the period under investigation, 800 cases of tuberculosis were reported, of which 68 (8.5%) cases were considered TB/HIV co-infected, based on positive records for anti-HIV serology. 62 cases (91%) of TB/HIV co-infection were geocoded, highlighting the majority occurrence in urban census sectors. A heterogeneous spatial distribution of cases was observed, with prevalence varying between 0.00 and 7.00 cases/100 inhabitants, with higher rates in the eastern region of the urban census sectors, and the Kernel estimator revealed hot areas with higher density of cases/ km2 in urban census tracts characterized by intense socio-spatial inequalities. Conclusions: Vulnerable territories, as well as the spatial heterogeneity of identified cases of TB/HIV co-infection, constitute support for management and health services, in the search for improving the management of these patients and reducing social and access inequities, through the implementation of effective health actions. |

Métodos: Estudo ecológico, descritivo e exploratório, realizado em Imperatriz (MA), Nordeste do Brasil. Os setores censitários do município foram considerados como unidades ecológicas de análises. Incluíram-se os casos de coinfecção TB/HIV, a partir das fichas de notificação de tuberculose registrados junto ao Sistema de Notificação de Agravos (SINAN) entre 2010 e 2019. Realizaram-se a análise espacial de área e análise de Kernel. Os casos foram geocodificados, por meio do software TerraView, versão 4.2.2, e ferramentas como Batch Geocode e GPS. Resultados: No período sob investigação, notificaram-se 800 casos de tuberculose, dos quais, 68 (8,5%) casos foram considerados coinfectados TB/HIV, mediante os registros positivos para sorologia anti-HIV. Foram geocodificados 62 casos (91%) de coinfecção TB/HIV, com destaque para ocorrência majoritária em setores censitários urbanos. Observou-se distribuição espacial heterogênea dos casos, com prevalência variando entre 0,00 e 7,00 casos/100 habitantes, com maiores taxas na região leste dos setores censitários urbanos, e o estimador de Kernel revelou áreas quentes com maior densidade de casos/km2 em setores censitários urbanos caracterizadas por intensas desigualdades socioespaciais.
