Introduction: Symptoms of Anxiety and Depression represent risk factors that have been associated with the incidence and progression of chronic diseases that can negatively impact the patients health-related quality of life (HRQoL). The development of modern statistical techniques and advanced information technologies have enabled the creation of software consisting banks of items, such as the Patient-Reported Outcomes Measurement Information System - PROMIS®, made up of more accurate physical and psychosocial domains to better evaluate the HRQoL. Objectives: To translate and culturally adapt to the Portuguese language items of the PROMIS® Anxiety and Depression bank of the domain Emotional Distress. Methods: The adaptation process of the banks Anxiety and Depression followed the FACIT methodology universal translation recommended by the developers of PROMIS®. The multiple steps were composed of Forward Translations, Reconciliation, Back-Translation, Quality Control: Review Back-Translation, Independent Reviews, Finalization Process, pretest and incorporation of the results of the pre-test in the process of translation. The 29 items of the bank Anxiety and the 28 items of the bank Depression were subjected to a linguistic revision made by a translator native of Portugal, whose purpose was to identify possible incompatibilities regarding language translations performed by Brazilian translators. Then, the pre-final version of each item was tested on a sample of ten patients, by means of interviews, retrospective and cognitive debriefing. Results: There was a need to rewrite three items of the bank of Anxiety and one of Depression, since the cognitive interviews conducted with patients showed loss of semantic equivalence in relation to the original items. The changes made in the pretested version reached the equivalence of meaning to the original scale, leading to a new version compatible with the language skills and cultural peculiarities of the Brazilian population. Conclusion: The process of translation and adaptation of the domain Emotional Distress originated items semantically and conceptually equivalent to the original. Final Thoughts: The items of the bank of Anxiety and Depression will enable a more accurate assessment of the impact of the signs and symptoms of Anxiety and Depression in chronic diseases, as well as an aid in the conduct of public policies and allocation of health resources. The development of banks will reduce the time onus that is on the patient and the number of samples in clinical trials. The banks of items in Anxiety and Depression will be available for use in the Brazilian population after checking the psychometric performance in patients with different types of chronic diseases.
