Estudo das representações de crianças internadas em hospital sobre o adoecimento e a hospitalização em uma abordagem piagetiana

Resumo: | The construction of knowledge by children and teenagers has instigated research in various areas. The social knowledge is constructed in the interaction between subject and the environment. Thus, children and teenagers having an experience of hospitalization construct knowledge from their experience. While hospitalized, the child is removed from its environment and its reality is suddenly transformed. Therefore, in addition to physical suffering arising from the disease, the child suffers from being separated from family, from his daily activities and the lack of what is happening with him. As a humanization strategy, the hospital playroom is used to minimize this suffering since the function of the hospital is not only cure diseases, but also to promote well-being and quality of life for patients. Thus, seeking to know what the hospitalized children think about the process of illness and hospitalization, some questions arise: What are the representations of illness and hospitalization that children who participate in recreational activities have in the hospital? What do these children think and feel to be sick and hospitalized? Thus, the aim of this study was to analyze the representations of illness and hospitalization from the perspective of children, 4-12 years, hospitalized in a medical institution that has recreational activities. The Piagetian clinical method was used as a technique for collecting and analyzing data. This way, we made a collection of data through the following steps: a) construction of the instrument for data collection and pilot study and b) data collection by clinical interview. Subsequently, the data were processed such that they were divided in six categories: hospital s physical environment, hospital s human environment, procedures and hospital routines, illness and healing, recreational practices and playroom. The analysis was performed after a joint study relating the categorized answers and the levels of responses in which subjects are in agreement with Piagetian theory. The results indicated that the construction of social knowledge goes through evolutionary stages, with the levels ranging from simple to complex. Our hypothesis that children develop representations of the process of illness and healing since very early was corroborated. However, it is important to emphasize that the same child has different levels of understanding according to the categories established. The categories that brought more negative aspects of experience as pain, waiting for service and better conditions of physical installation achieved higher levels of understanding hen compared with categories that were not related to suffering, e.g. to play in the hospital. It is important to know the representations of the subject so that we can review hospital procedures and even humanizing practices already developed. When we listened to the children, we realize that the hospital environment is not prepared to receive children, because the environment and the hospital practice only reflect the needs of adults, without regard that children have other needs in terms of both space, furniture and human treatment: children have the right to be heard, are entitled to know what's going on with you and must have their children's needs respected. The possibility of playing is a step forward that enables the hospital to be not merely an entirely bad reminiscence in his memoirs. |

O conhecimento social é construído na interação entre o sujeito e o meio em que vive; assim, crianças e adolescentes que passam por uma experiência de hospitalização constroem conhecimento a partir de sua vivência. Quando hospitalizada, a criança é afastada de seu meio e sua realidade sofre alterações e transformações. Desta forma, além do sofrimento físico advindo da doença, a criança sofre por estar separada dos familiares, das atividades cotidianas e pelo desconhecimento do que está acontecendo consigo. Para minimizar este sofrimento são utilizadas estratégias de humanização hospitalar, como a brinquedoteca hospitalar, uma vez que a função do hospital não é apenas curar doenças, mas também promover bem-estar e qualidade de vida aos pacientes. Assim, ao buscar conhecer o que crianças hospitalizadas pensam sobre o processo de adoecimento e internação hospitalar, surgem algumas questões: Quais são as representações sobre o adoecimento e hospitalização de crianças que participam de atividades lúdicas no ambiente hospitalar? O que estas crianças pensam e sentem ao estar doentes e hospitalizadas? A fim de responder a estas questões, o objetivo desta pesquisa foi analisar as representações sobre o adoecimento e a hospitalização pela perspectiva de crianças, de 4 a 12 anos, hospitalizadas em uma instituição hospitalar que possui atividades lúdicas. Diante da natureza do problema abordado, utilizamos o Método Clínico Piagetiano, como método de coleta e análise dos dados, visando alcançar o objetivo proposto. Para isso empreendemos uma coleta de dados através das seguintes etapas: construção do instrumento de coleta de dados e realizaçao do estudo piloto, e, coleta de dados por meio da entrevista clínica. Após realizadas as entrevistas, realizamos o tratamento dos dados elaborando seis categorias: ambiente físico hospitalar, ambiente humano hospitalar, procedimentos e rotina hospitalar, adoecimento e cura, práticas lúdicas e brinquedoteca hospitalar. Posteriormente realizamos a análise dos dados, de acordo com os níveis de respostas em que os sujeitos se encontram, de acordo com a teoria piagetiana. Os resultados apontaram que a construção do conhecimento social passa por fases evolutivas, indo de níveis mais simples até os mais complexos. Nossa hipótese de que as crianças desenvolvem representações sobre o processo de adoecimento e cura, desde muito jovens, foi corroborada. Mas é importante ressaltar que uma mesma criança possui representações e diferentes níveis de compreensão, de acordo com as categorias estabelecidas. Categorias que traziam aspectos vivenciados mais negativos, como dor, espera por atendimento e más condições de instalação física, alcançaram níveis maiores de compreensão, do que categorias que não estavam relacionadas com sofrimento, por exemplo, em relação ao brincar no hospital. Conclui-se que é importante conhecer as representações das crianças para que possamos rever os procedimentos hospitalares e até mesmo as práticas humanizadoras já desenvolvidas. Ouvindo as crianças percebemos que o ambiente hospitalar não é preparado para receber crianças, pois o ambiente e a prática hospitalar refletem as necessidades adultas, sem atentar-se que as crianças têm outras necessidades tanto em relação ao espaço físico e mobiliário, quanto à forma de tratamento humano: as crianças têm direito a ser ouvidas, tem direito de saber o que está acontecendo consigo e tem de ter suas necessidades infantis respeitadas. A brinquedoteca hospitalar é um avanço, pois permite que as crianças brinquem, tenham sua auto-estima aumentada e vivenciem momentos prazerosos no hospital.
