Bundle for prevention of urinary tract infection in Adult ICU: a literature review

Resumo: There are several infections related to health care (HAI) associated with different procedures, however, Urinary Tract Infections (UTI) draw attention because they are one of the most prevalent, despite the great preventive potential at the time of catheterization. In order to improve care and the care process, the concept of bundle emerges, inserted in good practices, which would be small well-evidenced intervention “packages” in order to ensure patient safety. The aim of this study was to review articles on the UTI prevention bundle of patients in intensive care. This is an integrative review using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) technique of research articles. We reviewed 20 articles that fit the criteria for research refinement, published between 1996 and 2016, with a predominance of 85% of production coming from the Southeast region. The main UTI preventive recommendations found were: hand washing before the procedure; maintenance of the closed system and the collection bag below the bladder; catheter insertion using aseptic technique and cleaning of the urethral meatus. It is then concluded that there is a reduced production of articles by the nursing team and inexpressive/nonexistent in other regions of the country.

Para mejorar la atención y el proceso asistencial surge el concepto de bundle, inserto en las buenas prácticas, que serían pequeños “paquetes” de intervención bien evidenciados para garantizar la seguridad del paciente. El objetivo de este estudio fue revisar artículos sobre el paquete de prevención de ITU de pacientes en cuidados intensivos. Esta es una revisión integradora que utiliza la técnica de artículos de investigación Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA). Se revisaron 20 artículos que se ajustan a los criterios de refinamiento de la investigación, publicados entre 1996 y 2016, con predominio del 85% de producción proveniente de la región Sudeste. Las principales recomendaciones preventivas de ITU encontradas fueron: lavado de manos antes del procedimiento; mantenimiento del sistema cerrado y de la bolsa colectora debajo de la vejiga; inserción de catéter mediante técnica aséptica y limpieza del meato uretral. Se concluye entonces que hay una producción reducida de artículos por parte del equipo de enfermería e inexpresiva/inexistente en otras regiones del país.Existem diversas infecções relacionadas à assistência à saúde (IRAS) associadas a diversos procedimentos, no entanto, as Infecções de Trato Urinário (ITU) chamam atenção por serem umas das mais prevalentes, apesar do grande potencial preventivo no momento da cateterização. Com a finalidade de aprimorar a assistência e o processo de cuidado dispõem que surge, inserido nas boas práticas, o conceito de bundle, que seriam pequenos “pacotes” de intervenção bem evidenciados com o intuito de garantir a segurança do paciente. O objetivo deste estudo foi revisar artigos sobre bundle de prevenção de ITU de pacientes em terapia intensiva. Trata-se de uma revisão integrativa através da técnica Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) de artigos de pesquisa. Foram revisados 20 artigos que se encaixavam nos critérios de refinamento da pesquisa, publicados entre 1996 a 2016 tendo predominância de 85% de produção procedente da região Sudeste. As principais recomendações preventivas de ITU encontradas foram: lavagem das mãos antes do procedimento; manutenção do sistema fechado e da bolsa coletora abaixo da bexiga; inserção do cateter em técnica asséptica e higienização do meato uretral.
