Pharmacological management of pain in palliative care: Contributions of the Clinical Pharmacist in a convergent care research

Resumo: Faced with the need to alleviate pain suffering in patients facing life-threatening diseases, the objective of this study was to develop a multiprofessional protocol for pain management in palliative care patients. This work is a qualitative research of a convergent care type, developed through the experience of a Pharmacist Resident, the Commission for Palliative Care and Terminality and the Pain Management Team of a teaching hospital in the state of Rio Grande do Sul. Furthermore, meetings were held to review, discuss and standardize the scales for pain assessment according to the care profile, like the selection of eligible drugs according to the World Health Organization's analgesic pain ladder. In addition, new drugs were included to guarantee greater variability in the therapeutic choice and the elaboration of standard prescriptions with the purpose of avoiding prescription errors, guaranteeing greater agility in the prescription process and the adherence to the protocol. After the protocol was elaborated, training was developed for the care teams and the hospital's clinical staff. Therefore, the clinical pharmacist played an extremely important role in the elaboration of the protocol by studying the pharmacotherapy of the eligible drugs in pain management, the rational use of these drugs, adequate management of adverse effects, assistance in the standardization of the scales for the evaluation of pain and participation in the training of care teams.

Se trata de una investigación cualitativa del tipo atención convergente, a través de la experiencia de un Residente de Farmacia, junto con la participación de la Comisión de Cuidados Paliativos y Terminalidad y el Equipo de Manejo del Dolor de un hospital escuela del interior de Rio Grande do Sul. Se realizaron reuniones para revisar, discutir y estandarizar las escalas para la evaluación del dolor según el perfil de atención, así como la selección de medicamentos elegibles según la escala analgésica del dolor de la Organización Mundial de la Salud. Además, se incluyen nuevos fármacos para garantizar una mayor variabilidad en la elección terapéutica y la elaboración de recetas estándar con el fin de evitar errores de prescripción, garantizando una mayor agilidad en la prescripción y en el cumplimiento del protocolo. Posterior a la elaboración del protocolo, se realizaron capacitaciones a los equipos asistenciales y al personal clínico del hospital. De esta forma, el farmacéutico clínico jugó un papel sumamente importante en la elaboración del protocolo al estudiar la farmacoterapia de los medicamentos elegibles en el manejo del dolor, el uso racional de estos medicamentos, el manejo adecuado de los efectos adversos, la asistencia en la estandarización de las escalas para la evaluación del dolor y la participación en la formación de equipos de atención.Diante da necessidade de amenizar o sofrimento da dor em pacientes que enfrentam doenças ameaçadoras à vida, o objetivo deste estudo foi elaborar um protocolo multiprofissional para o manejo da dor em pacientes sob cuidados paliativos. Trata-se de uma pesquisa de caráter qualitativo do tipo convergente assistencial, mediante a vivência de uma Residente de Farmácia, juntamente com a participação da Comissão de Cuidados Paliativos e Terminalidade e da Equipe de Gerenciamento da Dor de um hospital de Ensino do Interior do Rio Grande do Sul. Realizou-se encontros para revisão, discussão e padronização das escalas para avaliação da dor conforme perfil assistencial, assim como, a seleção dos medicamentos elegíveis de acordo com a escada analgésica da dor da Organização Mundial da Saúde. Além disso, incluiu-se novos medicamentos para garantia de maior variabilidade na escolha terapêutica e elaboração de prescrições padrões com propósito de evitar erros de prescrições, garantir maior agilidade na prescrição e na adesão ao protocolo. Após elaborado o protocolo, realizou-se capacitações às equipes assistenciais e ao corpo clínico do hospital.
