Main special orthopedic tests for semiological evaluation of the knee: a narrative literature review

Resumo: Introduction: biomechanical tests were developed in medicine so that semiology and propaedeutics could begin to identify impasses related to the patellofemoral joint, these were called orthopedic knee tests, which can be associated with various pathologies that are routine in the life of the orthopedist and that save time, money and psychological distress for the patient. Objective: to present concrete data about which semiological exams are the main and most relevant in clinical practice so far for the orthopedic evaluation of the knee. Methodology: this is a narrative literature review. The research was carried out through online access to the Google Scholar, Virtual Health Library (BVS) and EBSCO Information Services databases. Results and discussion: the literature was controversial in indicating a "gold standard test for the physical examination of the knee" for the diagnosis of meniscus and ligament injuries that affect this joint, this is related to the difficulty in standardizing the tests performed, which results in indices of broad accuracy. As an example of this, we have the divergences between studies that consider only clicks as a positive result in the McMurray test, while other authors establish that there is a need for a click and pain report by the patient for the test to be, in fact, positive. In general, the authors agree that physical examination maneuvers, despite being an excellent method for the initial assessment of the patient, do not present great diagnostic value, demanding other elements for a more accurate and reliable diagnosis that allows surgical interventions, such as imaging exams. Conclusion: good technique must consider each part being evaluated and also the sensitivity of the test. It is extremely important that you assess the opposite knee to obtain a parameter for the patient being examined. Although the imaging tests are quite complete and detailed, the physical examination of the knee is the most reliable for obtaining a more accurate diagnosis.

Objetivo: presentar datos concretos sobre qué exámenes semiológicos son los principales y más relevantes en la práctica clínica hasta el momento para la evaluación ortopédica de la rodilla. Metodología: se trata de una revisión narrativa de la literatura. La investigación se realizó a través del acceso en línea a las bases de datos de Google Scholar, Virtual Health Library (BVS) y EBSCO Information Services. Resultados y discusión: la literatura fue controvertida al indicar un “test estándar de oro para el examen físico de la rodilla” para el diagnóstico de las lesiones de meniscos y ligamentos que afectan a esta articulación, esto se relaciona con la dificultad de estandarizar los exámenes realizados, lo que da como resultado en índices de amplia precisión. Como ejemplo de ello tenemos las divergencias entre estudios que consideran únicamente los clics como resultado positivo en el test de McMurray, mientras que otros autores establecen que es necesario un informe de clic y dolor por parte del paciente para que el test sea, en hecho, positivo. En general, los autores coinciden en que las maniobras de exploración física, a pesar de ser un excelente método para la valoración inicial del paciente, no presentan gran valor diagnóstico, exigiendo otros elementos para un diagnóstico más certero y fiable que permita intervenciones quirúrgicas, como los exámenes imagenológicos. . Conclusión: una buena técnica debe considerar cada parte que se evalúa y también la sensibilidad de la prueba. Es extremadamente importante que evalúe la rodilla opuesta para obtener un parámetro para el paciente que está siendo examinado. Aunque las pruebas de imagen son bastante completas y detalladas, la exploración física de la rodilla es la más fiable para obtener un diagnóstico más certero.Introdução: os testes biomecânicos foram desenvolvidos na medicina para que a semiologia e propedêutica começasse a identificar impasses relacionado à articulação femuropatelar, esses foram denominados testes ortopédicos do joelho,os quais podem-se associa-los a diversas patologias que são rotineiras na vida do ortopedista e que poupam tempo, dinheiro e desgaste psicológico do paciente. Objetivo: apresentar dados concretos acerca de quais exames semiológicos são os principais e mais relevantes na prática clínica até o momento para avaliação ortopédica do joelho. Metodologia: trata-se de uma revisão narrativa de literatura. A pesquisa foi realizada através do acesso online nas bases de dados Google Scholar, Biblioteca Virtual em Saúde (BVS) e EBSCO Information Services. Resultados e discussão: a literatura foi controversa em indicar um “teste padrão ouro para o exame físico do joelho” para o diagnóstico das lesões meniscais e ligamentares que acometem essa articulação, isso está relacionado à dificuldade em padronizar os testes realizados que resulta em índices de acurácia amplos. A exemplo disso, temos as divergências entre estudos que consideram apenas estalidos como resultado positivo no teste de McMurray, enquanto outros autores estabelecem que há necessidade de estalido e relato de dor por parte do paciente para que o teste seja, de fato, positivo. De um modo geral, os autores concordam que as manobras do exame físico, apesar de serem um excelente método para avaliação inicial do paciente, não apresentam grande valor diagnóstico, demandando de outros elementos para um diagnóstico mais preciso e confiável que permita intervenções cirúrgicas, como os exames de imagem. Conclusão: a boa técnica deve considerar cada parte que está sendo avaliada e também a sensibilidade do teste. É extremamente importante que avalie o joelho oposto para obter um parâmetro para o paciente que está sendo examinado.
