Guidelines for the pediatric patient with acute lymphoid leukemia in outpatient monitoring: toxicity profile and adherence to treatment

Resumo: In the childhood population, an annual incidence of about 200,000 new cases of cancer is estimated worldwide, with leukemia being the most common type. With regard to nursing, this scenario demands the need for specific training of professionals working in pediatric oncology, in order to ensure safe and effective care. Given this scenario, taking into account the effects of treatment, the objective was to conduct guidelines after the medical consultation, reinforcing the most important information, aiming at the planning of an individualized care for pediatric patients with ALL in outpatient follow-up. The research consists of a longitudinal descriptive study. Evaluations of sixteen children were performed, fifteen of them with diagnosis of B-ALL and one with T-ALL, according to inclusion criteria, these are preliminary results. Regarding the most frequent toxicities, we must emphasize hematological as the most frequent and expected, taking into account the period of lower hematological cell count as a consequence of the drugs infused in the protocol BFM 2002 and GBTLI 2009. Other toxicities such as dermatological, gastrointestinal, hepatic and renal diseases should awaken close attention of professionals who are caring for the pediatric oncology patient. Another important aspect is the latent need of the nurses professionals to know and to master the graduation of the toxicities, guiding the care and the orientations from the classification of the CTCAE. The nurse should strive to elucidate and reinforce the information given by the physician and provide more information to the child and the family. It is also worth emphasizing that nurses' actions must be beyond practice and procedures, and that guidelines are as important as doing so in the search for an individualized, quality, humanistic and interdisciplinary care.

En lo que se refiere a la enfermería, ese escenario demanda la necesidad de capacitación específica de los profesionales que actúan en el área de oncología pediátrica, para asegurar un cuidado seguro y eficaz. En este contexto, teniendo en cuenta los efectos del tratamiento, el objetivo fue realizar orientaciones después de la consulta médica, reforzando las informaciones más importantes, buscando la planificación de una asistencia individualizada para pacientes pediátricos con LLA en seguimiento ambulatorial. La investigación consiste en un estudio longitudinal descriptivo. Se realizaron evaluaciones de dieciséis niños, siendo quince de ellas con diagnóstico de LLA-B y una con LLA-T, según criterios de inclusión, estos son resultados preliminares. En lo que se refiere a las toxicidades más presentadas, debemos resaltar las hematológicas como las más frecuentes y esperadas, teniendo en cuenta el período de menor conteo de células hematológicas como consecuencia de las drogas infundidas en el protocolo BFM 2002 y GBTLI 2009. Otras toxicidades como las dermatológicas, gastrointestinales, hepáticas y renales deben despertar mirada atenta de los profesionales que están prestando cuidado al paciente oncológico pediátrico. Otro aspecto importante es la latente necesidad de los profesionales enfermeros conocer y dominar la graduación de las toxicidades, orientando los cuidados y las orientaciones a partir de la clasificación del CTCAE. El enfermero debe esforzarse en dilucidar y reforzar la información dada por el médico y ofrecer más información al niño ya la familia. También cabe reforzar que las acciones del enfermero deben estar más allá de la práctica y de los procedimientos, y que las orientaciones son tan importantes como lo hacen en la búsqueda de una asistencia individualizada, de calidad, humanística e interdisciplinaria.Na população infantil, estima-se uma incidência anual de cerca de 200 mil novos casos de câncer em todo o mundo, sendo a leucemia o tipo mais comum. No que se refere à enfermagem, esse cenário demanda a necessidade de capacitação específica dos profissionais que atuam na área de oncologia pediátrica, de modo a assegurar um cuidado seguro e eficaz. Diante desse cenário, levando em consideração os efeitos do tratamento, o objetivo foi realizar orientações após a consulta médica, reforçando as informações mais importantes, visando o planejamento de uma assistência individualizada para pacientes pediátricos com LLA em acompanhamento ambulatorial. A pesquisa consiste em um estudo longitudinal descritivo. Foram realizadas avaliações de dezesseis crianças, sendo quinze delas com diagnóstico de LLA-B e uma com LLA-T, conforme critérios de inclusão, estes são resultados preliminares. No que se refere às toxicidades mais apresentadas, devemos ressaltar as hematológicas como as mais frequentes e esperadas, levando em consideração o período de menor contagem de células hematológicas como consequência das drogas infundidas no protocolo BFM 2002 e GBTLI 2009. Outras toxicidades como as dermatológicas, gastrointestinais, hepáticas e renais devem despertar olhar atento dos profissionais que estão prestando cuidado ao paciente oncológico pediátrico. Outro aspecto importante é a latente necessidade dos profissionais enfermeiros conhecerem e dominarem a graduação das toxicidades, norteando os cuidados e as orientações a partir da classificação do CTCAE. O enfermeiro deve empenhar-se em elucidar e reforçar as informações dadas pelo médico e oferecer mais informações à criança e à família.
